The North Carolina Department of Transportation is scheduled to replace Bridge #1066 with a new
bridge at the sanie IocaYion. I have included the Attachments G& L with a project description and a set
of the project plans. A stormwater management plan is also attached. Please review this project for
compliance by your Division.
We plan to begin construction as soon as possible. If further information is required, please contact Jerry

2009. This project meets the documentation requirements and approval procedures
under NEPA, as defined by FHWA, for Low Impact Bridge Replacements. This project
meets the standards of NCDOT's Minimum Criteria Rules.

Project Description — The project consists of replacing Bridge #106 over Burkes Creek on SR
1156 (Oakview Loop Road) and improving roadway approaches. The existing 85' L X 20' W
reinforced concrete deck girders on reinforced concrete bents and end bents bridge is
structurally deficient and will be replaced with a new, two span 120' L X 30' W, cored siab bridge
at the same location. An off-site detour will be utilized during construction. The project involves
no wetland impacts (there are no wetlands on the project site) and 16 linear feet of permanent
stream impacts associated with stream bank stabilization as well as 20 linear feet of temporary
construction impacts. While here are deck drains on the bridge, they will discharge onto deck
drain dissipator pads in the upland areas (i.e. not directly into the stream). Stormwater runoff will
be captured and discharged onto a dissipator pad at non-erosive velocities (0.1 fps). The
USFWS was solicited for comments on a series of Division 7 bridges in 2012. Per the attached

letter, the USFWS had no comments and did not plan to attend any of the field scoping
meetings. Please note that the Attachment G form lists the T&E status as "unresolved." The
Bald Eagle is protected in every county in North Carolina under the Bald and Gold Eagle
Protection Act; however, no suitable feeding/water source is located within one mile of the
project study area to support Bald Eagle habitat. Further, Mr. Gary Jordan (USFWS) recently
released the Programmatic Conference Opinion (PCO) for the recently listed Northern Long
Eared Bat (NLEB). This opinion, states in part, that "...it is the Service's conference opinion that
NCDOT activities in eastern North Carolina (Divisions 1-8), as proposed, are not likely to
jeopardize the continued existence of the NLEB."
