On March 20, 2023, the Division of Water Resources Winston-Salem Regional Office
received the following report of a discharge of biosolids from the abovementioned
permitted facility into surface waters :

Per Permit Condition 11.1 the Permittee shall maintain and operate the subject residuals
management program so there is no discharge to surface waters, nor any contravention
of groundwater or surface water standards. The above incident was a violation of the
referenced permit condition as biosolids bypassed from the storage structure and entered
the Deep River, a Class C water of the State.
