I will need ID (001, 002, etc), a short description, latitude/longitude (in degrees, minutes, seconds, please — not decimal
format), and receiving waterbody info. It's fine to indicate that receiving water is the same as any existing outfalls in our
system.

Thank you for updating this information.
In regards to some of the information on the form, our owner is not Hitoshi Nakamura. He no longer works at Bonset
America Corporation. I thought that I previously recalled that the owner contact name had "Bonset America
Corporation" in the field. If this is required to be an individual, can it please be changed to Shinji Takahashi? This was the
intent of changing the owner affiliation.
Also, I need to update the outfalls as well. How do I go about doing that? Is there a form that needs to be filled out with
the information or do I just need to send the information by email? Do I need to have any supporting documentation?
I appreciate your help and any information you can provide.
