This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US Environmental Protection Agency dated December 6, 1983. Please note that if a more environmentally sound alternative is available, the Division will require removal of the discharge from your facility. An engineering report, evaluating connection of your facility to the Greensboro sewer system, must be submitted to the Division by April 12, 1997. Upon review of the engineering report, the Division may reopen and modify this permit (see Part III, sectic) 1 E.). If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 27447, Raleigh, North Carolina 27611-7447. Unless such demand is made, this decision shall be final and binding. Please take notice this permit is not transferable. Part II, E.4. addresses the requirements to be followed in case of change in ownership or control of this discharge. This permit does not affect the legal requirements to obtain other permits which may be required by the Division of Environmental Management or permits required by the Division of Land Resources, Coastal Area Management Act or any other Federal or Local governmental permit that may be required. Continue to operate a 0.03 MGD wastewater treatment facility consisting of a flow splitter, a flow measurement and recording device, two aeration tanks, dual clarifiers, aerated sludge holding tanks, blowers, tertiary filtration, tablet chlorination, chlorine contact tanks, diffused reaeration, and a stand-by generator located at Monroe's Mobile Home Park, 955 Halsbrook Road, south of Greensboro, Guilford County (See Part III of this permit), and 2. Alternatives Analysis Condition Removal of the discharge will be required if a more environmentally sound alternative is available. An engineering report evaluating connection to the Greensboro sewer is due within 12 months of the issuance of this permit. Upon review of the results of the engineering report, the Division may reopen and modify this NPDES permit to require removal of the discharge, modified treatment designs, and/or revised effluent limitaitons within a specified time schedule. Special Condition(s): None. Permits & Engineering Comments: P&E recommends renewal of NPDES permit with current limits except for ammonia to be reduced as recommended above. Monitoring data suggest that the facility can consistently meet the 2/4 mg/l limits. Per conversation with Thomas Monroe and Susan Robson, the application will be completed and mailed to P&E as soon as possible. The wrong expiration date was listed on the NUCC database and has been corrected. _l Prepared by: GA UXO Regional Office Evaluation and Recommendations: April 8, 1996 MEMORANDUM To: Permit File From: Susan Robson Subject: NC0055913 Monroe's MHP Discussion with Lee Spencer about DEH concerns On April 4, 1996, Lee Spencer and I had a discussion about DEH's concerns regarding the subject facility. Their concerns are outlined in a memo dated March 26, 1996. The first question was requesting a reason why the permit for this facility did not contain the "zero flow" special condition like the permit for Woodlake MHP did. I checked into that and found that the subject facility discharges into a stream that has a positive 30Q2, whereas Woodlake discharges in a stream that is 0 7Q10 and 30Q2. Mr. Spencer was not aware of that. The other main question concerned any requirements that might be placed in the subject facility's permit regarding connection to a POTW. In a memo dated March 28, 1996, Ron Linville of the Winston-Salem Regional Office requested that a special condition be added to the final permit requiring the permittee to submit an alternatives report within 12 months of issuance of this permit. This report should include an analysis of connecting the facility to the City of Greensboro's new sewer line and pump station, and if this is a feasible option, the permittee should proceed with connection without delay.
