Subject: Request for a Nationwide Permit 3 and 401 Water Quality Certification for the
replacement of 2 culverts on NC HWY 12 in Salvo, Dare County. WBS # 1.1028111
The NC Department of Transportation is requesting the issuance of a Nationwide Permit
3 and 401 Water Quality Certification for the replacement of 2 culverts on NC HWY 12 in Salvo,
Dare County. The culverts are located on NC HWY 12 approximately 300 feet north of the
intersection with SR 1433. The culverts are located on a jurisdictional stream to the Pamlico
Sound, which is classified by DWR as class "SA;HQW" waters. The existing culverts are (2) 18"
diameter X 88' long corrugated metal pipes. These culverts will be removed using NCDOT's
guidelines for culvert removal. The proposed culverts are (2) 24" diameter X 90' long corrugated
aluminum pipes and will be placed on the existing alignments as shown in the permit drawings.
The new culverts will be placed more than 1 foot below the surveyed stream bed. The project
will result in two linear feet of stream impact and no jurisdictional wetland impacts. The site is
also within an Area of Environmental Concern (AEC), and a CAMA General Permit .2300 will
be requested from the NC Division of Coastal Management.

Replace (2) 18"X88' CMP's under NC HWY 12. The existing pipes are failing.

The existing culverts will be replaced with (2) 24"X90' CAP's. Once the pipes are in place and back filled, a new roadway
surface and grassed shoulder will be established. The banks will be stabilized following NCDOT erosion control BMP's. The
work will be accom lished with excavator, backhoe, and dum trucks.
