The press release shall be issued within 24 hours after the owner or operator has determined that the discharge has reached surface waters of the State. In the event of a discharge of 15,000 gallons or more of untreated wastewater to the surface waters of the State, publish a notice of the discharge in a newspaper having general circulation in the county in which the discharge occurs and in each county downstream from the point of discharge that is significantly affected by the discharge. The Regional Office shall determine which counties are significantly affected by the discharge and shall approve the form and content of the notice and the newspapers in which the notice is published. Have there been other failures of this line in the past five years? ❑ Yes ❑ No ❑NA ❑ NE If so, then describe Form CS-SSO Fnge 12 System Visitation ORC ❑ Yes Backup Name: Dennis Sumpter Certification Number: 1010990 Date visited: 3/17/2023 Time visited: 6:00am How was the SSO remediated (i./e. Stopped and cleaned up)? Metal grate fell into plant drainage system creating a partial clog. Staff created a dam to hold water in the process drainage area and pushed water into the adjacent trench being sent to wastewater treament. Before the dam was created, a limited amount escaped into the surrounding area. A vacuum truck was used to remove all sitting water; area was flushed with clean water and vacuumed up. All released process water was collected and sent to wastewater. No water left the site. Grating was removed from drainage ditch and operation was returned to normal.
