LP has operated in compliance with the permit requirements during 2022 except as noted below. Groundwater Monitoring — Monitoring,Report Keeping, and Reporting LP has six (6) monitoring wells listed in Permit No. WQ0000461. These wells were sampled twice (2) per year according to the requirements of the current permit. The six (6) wells are located on LP property and correspond to selected groups of land application fields that are monitored. All land application ceased in the monitoring area of field 1228-10387-25 in 2008. LP has indicated non-compliance with this requirement due to elevated nitrate readings exceeding 10 mg/l measured in groundwater monitoring wells DSI-MWI, DSI-MW5, and DST-MW9 during the March and September monitoring events. The results of the groundwater monitoring are included in this report in the Groundwater Analysis section. 1668 ABTCO Road ADDRESS North Wilkesboro, NC 28669 336.696.2751 TEL 336.696.3443 FAX www.lpcorp.com WFB DMSDF 1068 ABTCO Road ADM= North Wilkesboro, NC 286S9 336.696.2751 .may 336.696.3448 FAX WWW-IpcorP_corrt WEE A U O E a O H Cr O H z O U 1❑ ❑ N F.'
