Dear Current tenant:
Enclosed is a copy of the Compliance Evaluation Inspection for the inspection conducted at the subject
facility on January 25, 2023, by Ori Tuvia. The permitee was not present on site at the time of the visit, and no
material was left for the inspector to review.
The following deficiencies were found during the inspection, and should be corrected:
1) No chlorine tablets were left for the inspector to review, chlorine tablets that are designed for
wastewater treatment (not swimming pools) must be used in the chlorine contact chamber. Due to
the permitee not being home the inspector could not examine the chlorine contact chamber.
2) Change of ownership on the permit is needed, form is attached.
3) Annual permit fees for the years 2021 and 2022 are past due, invoices are attached.
4) Records of sampling (flow permitting) and septic tank being pumped should be kept to be reviewed
for future inspections. The effluent sample should be taken annually (last known sampling event was
done on 9/26/2018), the septic tank must be pumped every 5 years (last known pumping was done
on 1/5/2016).
