I acknowledge receipt of your Notice of Violation & Intent to Issue Civil Penalty regarding
an SSO event on March 26, 2021. The NOV, dated 6/17/2021, was received 6/28/2021.
--The following -narrative -and -attachments. have -been -included -to -explain -the cause of -this.-- -
event (power outage/generator failure) and the actions being taken to prevent future
overflows.
Narrative - Incident Number 202101089: Friday, March 26, 2021
The Weant Road Pump Station (PS) has been undergoing many upgrades in recent years
and there is a light at the end of this tunnel. After a year of waiting for a new MCC to
arrive, an electrical safety and improvement project was nearly complete. However, a
setback occurred in the early morning hours of Friday, March 26th
Following a night of heavy rain, a power outage was encountered. Weant Road PS is well
equipped to handle outages with a large generator and ATS on site. They get
preventative maintenance annually and have worked without issue when called upon

before. On this night, however, something failed in the generator, and it never called for
power resulting in an SSO.
The City of High Point monitors a SCADA system for Weant Road PS. In the early
morning hours of Friday, March 26rh, High Point staff called Archdale Public Works staff to
advise they were not getting readings and presumed a power outage. Staff from both
entities arrived to find the station was not running. The power was out, and the generator
had failed to turn on. They manually started the generator once the situation was deemed
safe. A large bypass pump was on hand as backup for upcoming scheduled PS
improvements. It was hooked up and manually started to help the PS catch up with the
flow. The overflow ended shortly thereafter.
When the sun came up, staff contacted the electrician involved with ongoing PS projects
and asked for assistance in identifying issues with the generator and/or the ATS.
Ultimately, an electrical board in the generator was identified as the source of the problem.
--Volvo parts were -ordered and installed by Carolina CAT at a cost of $23,906.09.
In accordance with the City of Archdale's Sewer Overflow Response Plan, staff cleaned
up the area when flooding receded, and things were safe. They spread hydrated lime and
then prepared a Public Notice and Press Release to send to our local newspapers.
Public Works staff deserve high marks for their efforts on this event. They responded in
the middle of the night and worked quickly to address the issue and get it resolved. They
then looked at long-term changes needed to avoid similar problems in the future. In
addition to the generator repair, it was determined that there were issues with the existing
ATS that could be problematic in the future. Staff asked management about the possibility
of replacing the ATS in the current fiscal year and were given a "Go". They then moved
forward with a contract on the ATS project at a cost of $22,550.
The City feels we have been taking appropriate actions to minimize the possibility for SSO
events at this critical PS for years.
• A $1.3M parallel force main was installed to take the pressure off of a singular force
main handling nearly all of the City's wastewater.
• Pump and generator PM work -occurs annually at all stations.
• A $282,000 electrical upgrade and safety project at this PS is coming to its end.
• . Pumps #1 and #2 (100HP) had their suction valves replaced for $39,175.22 in
February 2021.
• A new, additional 250HP pump is programmed in the FY22 budget and will serve
as additional insurance during high flow events/pump failures at this location.
• The gravity line feeding into the PS is budgeted for upsizing and adding valve
functionality in FY22.
• The wet well at the PS is budgeted for a rehab in FY22.
• In case of possible 100HP pump failure in the new year, the FY22 budget has funds
to replace the 100HP pump with a 125HP pump if necessary.
• The City was awarded a sewer AIA grant from DEQ which will be completed in
FY22 to assess the system and prioritize needed repairs/upgrades.

A review has been conducted of the self -reported Sanitary Sewer Overflows (SSO's) 5-Day Report/s submitted by
City of Archdale. The Division's Winston-Salem Regional Office concludes that the City of Archdale violated Permit
Condition I (2) of Permit No. WQCS00312 by failing to effectively manage, maintain, and operate their collection
system so that there is no SSO (Sanitary Sewer Overflow) to the land or surface waters and the SSO constituted
making an outlet to waters of the State for purposes of G.S. 143-215.1(a)(1), for which a permit is required by Ij
G.S. 143-215.1.
The Winston-Salem Regional Office is providing the City of Archdale an opportunity to provide, evidence and
justification as to why the City of.Archdale should not be assessed.a civil penalty for the violation(s) that are

This Notice of Violation / Notice of Intent to Enforce (NOV/NOI) is being issued for the noted violation. Pursuant
to G.S. 143-215.6A, a civil penalty of not more than twenty-five thousand dollars ($25,000.00) may be assessed
against any person who violates or fails to act in accordance with the terms, conditions, or, requirements of any
permit issued pursuant to G.S. 143-215.1.
This office requests that you respond to this Notice, in writing, within 10 business days of its
receipt. In your response, you should address the causes of non-compliance, remedial actions, and all other
actions taken to prevent the recurrence of similar situations. The response to this correspondence will be
considered in this process. Enforcement decisions will also be based on volume spilled, volume reaching surface
waters, duration and gravity, impacts to public health, fish kills or recreational area closures. Other factors
consider determining the amount of the civil penalty are the violator's history of non-compliance, the cost of
