According to correspondence with Charles Weaver of the Division of Water Resources, the Short Form C-
GW is no longer accepted for renewal submittals. Our understanding is that forms EPA -Form 1, Form 2E
and Form 2C with Tables A, B, and C from EPA Form 2C are required. Further, AECOM understands that
there are no special analytical requirements for this permit renewal. Please contact us with any further
