Box 77013 Greensboro, N.C. 27417-7013 SUBJECT: Amendment to Permit No. NC0023299 Authorization to Construct Woodlake Mobile Home Park Wastewater Treatment Facilities Guilford County Dear Mr. Hart: A letter of request for an Authorization to Construct was received April 29, '987, by the Division and final plans and specifications for the subject project have •een reviewed and found to be satisfactory. Authorization is hereby granted for the modifications including increasing the total aeration tank volume from 35,000 gallons to 70,000 gallons, and repiping the two existing 5000-gallon tanks under the sand filters to include aeration and serve as aerobic digesters. This Authorization to Construct is issued in accordance with Part III paragraph C of the NPDES Permit No. NC0023299 issued August 7, 1985, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No. NC0023299. The Permittee must employ a certified wastewater operator in accordance with Part III paragraph D of the referenced permit. The sludge generated from these treatment facilities must be disposed of in accordance with G. S. 143-215.1 and in a manner approvable by the North Carolina Division of Environmental Management. The Winston-Salem Regional Office, telephone number 919/761-2351, shall be notified in advance of operation so that an in -place inspection can be made. Such notification to the Regional Supervisor shall be made during the normal office hours from 8:00 a.m. until. 5:00 p.m. on Monday through Friday, excluding State Holidays. The septic tank must be pumped once a year and the filters must be rehabilitated as needed, but should be inspected no less than once every three years of operation. Pollution Prevention Pays P.O.
