Keith Adams, President Tarheel Sand & Stone, Inc. 1108 Tuckda Way Hamptonville, NC 27020 Subject: Tarheel Sand & Stone, Inc. — Mine #52 In -Stream Sand Mining Operation NPDES Permit No. NCG520049 Davie County Dear Mr. Adams: Donald R. van der Vaart Secretary This letter is just to confirm the recent conversation that Mike Mickey of this office had with you on October 20, 2015, concerning the Tarheel Sand & Stone in -stream sand mining operation (Mine #52) located off Hwy 64 in Davie County. Based on the conversation, it is our understanding that this site has not operated since July 2015. This facility has a single settling pond for water that is pumped through the screen and routed back to the Yadkin River via an open ditch. Observations at the site on October 22, 2015 noted that the settling pond had been cleaned out and that the river bank was sufficiently vegetated. Please note that the General Permit expired on July 31, 2015. The Division is currently in the process of renewing the permit. A new Certificate of Coverage will be mailed to you in the near future. Additionally, our database shows that your annual fee payment is up to date. Should this in -stream sand dredging operation be re -activated in the future, the permit will require a semi-annual flow estimate as well as the installation of warning signs on each side of the river upstream of the operation to warn canoeists and other recreational water users. No standardized discharge monitoring forms have to be submitted. The records must simply be maintained for the facility for a period of three years. Should you have any questions concerning this letter, please contact Mike Mickey or me at (336) 776-9800. Sincerely, cc: WSRO r. A Sherri V.
