Mr. Tim Church, ORC, was present for the inspection. The inspection is comprised of an overall evaluation of the facilities equipment, capability to meet effluent limits, and compliance with the NPDES permit. Permit Your NPDES permit became effective on April 1, 2011 and expires on March 31, 2016. It is recommended that the permittee register for eDMR. It will be a permit requirement in your next permit cycle. Please remember to submit your NPDES permit renewal application EPA Form D. Facility Site Review The equipment, grounds, and circular sandfilter appeared to be in good condition. Flow Measurement There is no flow. In the event the facility will be restarted, you will be required to verify and calibrate the flow measuring device within 10% accuracy. Laboratory There are no tests performed. Effluent/Receiving Waters There was no effluent discharge to Dog creek. Operations & Maintenance There is minimal operation & maintenance required since the facility is not in operation. In the event of a plant restart, a back-up ORC of the appropriate grade will be required. Self -Monitoring Program A review of the monthly self -monitoring reports from January 2015 through August 2015 reported no flow. The monitoring reports are correctly submitted. Sludge Disposal There is no sludge generated because the plant receives no wastewater. The compliance evaluation inspection is considered satisfactory.
