The North �arolina Departmen� of Transporta�ion is scheduled ta repla�� Brfdge #66� w�th a
carrugated steel pipe arch at the same location. The existing bridge is struct�.rally d�ficien�e �T�
wi11 also need to extend and existing 3 6" drivevv�ay pipe so that tributary flc�� will discharge
upstream of the new proposed headwall lc�cation (se� plan sheet). 11�I�ii�ation will b� pr�v�d�d
through EEP (70'�. No on-site detour is proposed. �tream impacts �.re tabulated belo�v.
Stream Impacts:
The replacement af the existing bridge with the proposed ne�%v culvert wi11 rec�uir� t�m.porary
dewatering. An impervious sa�.dbag dike will be �on�truct�d approxirnately 20 feet ups�tream c�f
the existing structure to create a small paol. Clean water w�ll tempora�ily b� pumped ���und c�r

IV). This praject was reviewed for the presence c�f threatened and endang�red sp�c�es �.�d na�.�

The proj ect impacts are not significant in nature ar�d should nc�t remc�ve or de��ade exis�ting wai�r
quallty uses. Stormwater will be routed through gra�s c�r rc�ck l�ned ditches, dependi�.g on th�
ditch line slope. Total 1ar�d disturbance w�ll be less than 1 acre4 Pipe siZes used w�ll be the
minimum necessary to create a safe roadway. By util�zing the eroslon cc�n�ral mea�ures c�utl�n�d
in the attachments to this application, downstream water qualliy standards w�ll be pr�t�cied. .�.11

Descrintion o1'review activities, results, and conclusions:
Review of HPO quad maps, HPO GIS infoi•mation, historic designations roster, and indexes was
undertaken on July 26, 2013. Based on this review, there are no existing NR, SL, LD, DE, or SS
properties in the Area of Potential Effects, which is 50'from the centerline each way. As this is a state
fi►nded project with no federal actions, it is subject to GS 121-12(a), which cequires consideration of only
National Register listed properties in the APE. There are no NR listed propei�ties in the APE, thej•efore
the project needs no architectural survey. Should the project acquire federal fiinding, permits, or other
involvement, or if its scope changes in any way, it will require additional review.

Project Descriplion: Grade, drain, base, and pave SR2020 (Sett/e Road) in Wi(kes County, North
Caro[ina. The average width of the subject roadway is 14ft and the proposed width is l8ft. Al[
improvements wi/l be conftned to the right-of-way (ROIij. The archaeologica/Area ofPotenda[
Effects (APE) encompasses the 0.60 mile road length and SOft wide existing ROW.
SUMMARY OF CULTiJRAL RESOURCES REVIEW
Brief description of revlew activitles, results ojrevlew, and concluslons:
A map review ahd site file search conducted at the Office of State Archaeology (OSA) on Thursday,
August 1, 2013 determined that no previously documented archaeological sites are located within the
boundaries of the archaeological APE or directly adjacent to the project study area. An inspection of
NaHonal Re$ister of Historic Places (NRHP), State Study Listed (SL), Locally Designated (LD),
Determined Eligible (DE), and Surveyed Site (55) properties employing resources available on the North
Carolina State Historic Preservation Office (NCSHPO) website concluded that no significant historic
properties with potential archaeological deposits are situated in the APE. In addition, historic maps of
Wilkes County and the project area were examined for evidence of Eormer structure locations, land use
pattems, or other evldence of occupation within or proximal to the APE. In general, the cultural resources
review confirmed that no existing NRI-II' listed properHes or significant archaeological sites will be
impacted by the proposed project.
Further, the APE was referenced on topographic, geologic and NRCS soil survey maps (FcB2, FcC2, FaD,
DpC2, CoA, RdE, FaD) to appraise environmental, geomorphological, hydrological, and other
mrrelatives that may have resulted in past occupation at this location. Aerial photographs (NCDOT
Spatial Data Viewer) were also examined and the Google Street View map application (when amenable)
was utilized for assessing disturbances, both natural and human induced, which compromise the
integrity of potential archaeological sites/deposits.
Brief Explanation of why the available informatlon provldes a reliable basls for reasonably predictinq
that there ore no unidentified hlstoric properties in the APE:
The project APE contains no NRHP listed historic properties nor previously identified archaeological
sites or cemeteries. According to soil data, the disturbed right-of-way is largely underlain by moderate(y
eroded soil rypes or characterized by sloping ground surfaces unlikely to harbor traces of past cultural
activity. Based on all of the above information, archaeological site potenHal is considered very poor.
Because the proposed project is diminutive in scope, lacks federal interaction, and will not impact any
undisturbed areas beyond the existing 50ft. wide ROW, meaningful NRHP eligible archaeological
