Thank you for pointing this out! I've gone ahead and submitted the ROS renewal requests through the link you
provided.
Brittany emailed me this morning and I believe she has already corrected the outfalls in the eDMR system.
For outfall 007 at NCG080063 (City of Charlotte Fleet Management), we don't have ROS, what we have is a documented
State inspection done by James Moore in 2018 where he advises that we can suspend monitoring at outfall 007 due to
dangerous site conditions in the area (sink hole). He advises that this suspension is in effect until site repairs are made
and the parking lot is back in service. The repairs still have not been made and I don't believe they will be made any
time soon (or at all). I've attached a PDF copy of the inspection report where this is documented.

I couldn't find a record of where you all submitted ROS renewal certifications along with the original ROS approval
documentation for these permits.
Those can be uploaded for each permit through this form below, and then I can add the outfalls as needed. The ROS
Renewal Certification is here. Please also upload the original ROS approval letter (I'm not finding it in either permit file,
unless I missed it in a historical scan).
I will also need the latitude and longitude (in degrees/min/sec, not decimal degrees) for each outfall so I can get them
entered. I will put monitoring suspensions on the ones that don't require monitoring because of ROS. You can email me
those coordinates, or just include a sheet with them listed with the upload below— I will process those submittals when
they come in, so I can take care of it.

Hey Brittany,
I noticed most of these outfalls had been added into eDMR, however, some are still missing or incorrect.
For Fleet NCG080063, it is still only showing outfall 001, when it should only show outfall 006. (001 should not be shown
since it has representative status)
For Light Vehicle Maintenance NCG080879 it is now showing 001 and 002, when it should be showing 003. (002 should
not be shown since it has representative status)
Please let me know if you are able to correct these or if you need anything additional from me to complete this request.

As discussed with Bethany and Kieu below, our City of Charlotte NCG08 facility outfalls are not listed correctly in your
system.
Each facility in the report only has outfall 001 on record, when actually we have multiple outfalls at most facilities. I
confirmed this in eDMR the other day when I went to submit our DMRs and only outfall 001 was available in the
dropdown menu.
I've attached a Word document that lists all of the outfalls and their coordinates at each of the City's NCG08 facilities.
Do you need anything else from us to complete this update request?

I've had a chance to review the Permit Summary Reports for those facilities and saw the owner information
changes. Thank you!
I did also notice some other changes we need to make to be up to date.
One of the big ones is the outfalls. Each facility in the report only has outfall 001 on record, when actually we have
multiple outfalls at most facilities. I confirmed this in eDMR the other day when I went to submit our August DMRs and
only outfall 001 was available in the dropdown menu.
Additionally we will need to remove some, and replace some relevant contact info. I've highlighted deletions in blue,
changes in green, and additions in yellow.
Is this something you are able to help with, or do we need to submit an electronic or hardcopy form?
Please let me know the best way to proceed.

Kieu can advise if there are additional forms, but I think the attached forms cover it. We just also need information
about what you want the owner entity to look like (name that includes specific department, address, etc.) and exactly

We're going to take care of this very soon but we'd like to get the department Director to sign all potential documents at
one time because it's kind of an ordeal to run things up the management chain. I'm thinking a signatory authority form
is one document because Andrew & I will be signing DMRs for his facilities. Can you think of anything else that he may
need to sign as permit owner?
