Dear Mr. Glass:
In accordance with your permit major modification request received October 12, 2022, and
subsequent additional information received January 10, 2023, we are forwarding herewith a modified
Attachment B certified January 19, 2023, to the City of Randleman for the subject residuals management
program.
Modifications to the subject Attachment are as follows:
➢ Addition of Field NC-RD-48 in Randolph County, consisting of two fields that total 40.13
acres.
➢ Removal of Fields NC-RD-03 and NC-RD-46 in Randolph County due to the transfer of these
fields to Permit No. WQ0001684, totaling 90.6 acres.
The modified Attachment B shall be effective from the date of certification until May 31, 2026
(i.e., expiration date of the current permit), shall replace Attachment B of Permit No. WQ0004825 issued
February 13, 2020, and shall be subject to the conditions and limitations as specified therein, as well as
those specified in the above -referenced permit. Please replace Attachment B of Permit No. WQ0004825
that was issued February 13, 2020 with the modified Attachment B. Please note that the Permittee shall
submit a renewal application no later than December 2, 2025.
Please note that only the maps for new land application sites are attached to this letter. It is the
Permittee's responsibility to ensure that a complete permit is created by inserting these documents into the
previously issued permit.
If any parts, requirements, or limitations contained in this certification are unacceptable, the
Permittee has the right to request an adjudicatory hearing upon written request within 30 days following
receipt of this permit. This request shall be in the form of a written petition, conforming to Chapter 150B
of the North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail
Service Center, Raleigh, NC 27699-6714. Otherwise, this certification shall be final and binding.
