Cover Letter: 1. No Comments B. Wastewater Irrieation System — Renewal (FORM: WWIS-R 02-21) Application: 1. Section III — Flow Information: The applicant marked the wastewater composition as 100% domestic. However, the "Plan of Action — Resolution of Wastewater Treatment Hydraulic Issues" seems to indicate that various unspecified users of Trinity American and various unspecified fixtures in the industrial facilities are contributing to the wastewater flows. Please clarify whether the flows from this facility are domestic, industrial, or a mixture of both. Attachment F — Demonstration of Future Wastewater Capacities: Prior to exceeding 70 percent of the system's permitted hydraulic capacity, the permittee shall submit an engineering evaluation of their future wastewater treatment, utilization, and disposal needs which outlines plans for meeting future needs by either expansion of the existing system, elimination or reduction of extraneous flows, or water conservation, and shall include the source of funding. Prior to exceeding 90 percent of the system's permitted hydraulic capacity, the permittee shall either obtain all needed permits for the expansion of the system and submit final plans and specifications for expansion (if construction is planned). In both cases, if expansion is not proposed then a justification shall be made based on past growth records and future growth projections that wastewater treatment needs will be met, including conservation plans or other measures to achieve waste flow reductions as relevant. The attached "Plan of Action — Resolution of Wastewater Treatment Hydraulic Issues" does not contain an engineering evaluation, a funding source, growth records, or growth projections for estimating future treatment, utilization, and disposal needs. Pursuant to 15A NCAC 02T .0118(1) and 15A NCAC 02T .0118(2), submit an updated plan of action containing that information. 3. Attachment L —Setback Waivers: The applicant checked "no", indicating that the existing permit has no setback waivers. However, the existing permit does contain setback waivers. Pursuant to 15A NCAC 02T .0506(d), provide a copy of all setback waivers that have been written, notarized, signed, and recorded. C. Other Documents: 1. Plan of Action — Resolution of Wastewater Treatment Hydraulic Issues: Please provide the —punch list' of fixture items that need to be updated and replaced" which is mentioned in this document to further support this plan.
