Enclosed is a report for the inspection performed on October 17, 2012 by Nick Jones. Where
finding(s) are cited in this report, a response is required. Within thirty days of receipt, please
supply this office with a written item for item description of how these finding(s) were corrected.
If the finding(s) cited in the enclosed report are not corrected, enforcement actions may be
recommended. For certification maintenance, your laboratory must continue to carry out the
requirements set forth in 15A NCAC 2H .0800.

This laboratory was inspected by a representative of the North Carolina Wastewater/Groundwater
Laboratory Certification (NC WW/GW LC) program to verify its compliance with the requirements of 15A
NCAC 2H .0800 for the analysis of environmental samples.
II. GENERAL COMMENTS:
The laboratory was clean and well organized. Records were filed in an orderly manner and readily available
for inspection. Proficiency testing samples have been analyzed for all certified parameters for the 2012
calendar year and the graded results were 100% acceptable. Current quality assurance policies for
Field laboratories and the approved procedures for the analysis of the facility's currently certified
parameters were provided at the time of the inspection.
Customized benchsheets with spaces for all required documentation was provided by the auditor
during the inspection.
Contracted analyses are performed by Environment 1, Inc. (Certification #10).
III. FINDINGS, REQUIREMENTS, COMMENTS AND RECOMMENDATIONS
Documentation
Comment: The meter calibration and calibration times were not being documented for Dissolved
Oxygen (DO). The NC WW/GW LC Approved Procedure for Field Analysis of Dissolved Oxygen states:
The calibration and time of calibration must be documented whenever sample analysis is performed.
Notification of acceptable corrective action (i.e., confirmation that the customized benchsheets with
spaces for all required documentation that was provided by the auditor during the inspection had been
implemented) was received by email on 10/29/12. No further response is necessary for this finding.
Comment: The analysis time of the check standard was not documented for Total Residual Chlorine
(TRC). The NC WW/GW LC Approved Procedure for Field Analysis of Total Residual Chlorine states:
The analysis time of the check standard must be documented whenever sample analysis is performed.
Notification of acceptable corrective action (i.e., confirmation that the customized benchsheets with

implemented) was received by email on 10/29/12. No further response is necessary for this finding.
Comment: The units of measure were not documented for pH, Temperature (Temp) or TRC. The North
Carolina Administrative Code, 15A NCAC 2H .0805 (g) (1) states: Data pertinent to each analysis must
be maintained for five years. Certified Data must consist of date collected, time collected, sample site,
sample collector, and sample analysis time. The field benchsheets must provide a space for the signature
or initials of the analyst, and proper units of measure for all analyses. Notification of acceptable
corrective action (i.e., confirmation that the customized benchsheets with spaces for all required
documentation that was provided by the auditor during the inspection had been implemented) was
received by email on 10/29/12. No further response is necessary for this finding.

A. Finding: The laboratory needs to increase the documentation of purchased materials and
reagents as well as documentation of standards and reagents prepared in the laboratory.
Requirement: All chemicals, reagents, standards and consumables used by the laboratory
must have the following information documented: Date Received, Date Opened (in use),
Vendor, Lot Number, and Expiration Date. A system (e.g., traceable identifiers) must be in
place that links standard/reagent preparation information to analytical batches in which the
solutions are used. Documentation of solution preparation must include the analyst's initials,
date of preparation, the volume or weight of standard(s) used, the solvent and final volume of
the solution. This information as well as the vendor and/or manufacturer, lot number, and
expiration date must be retained for chemicals, reagents, standards and consumables used for
a period of five years. Consumable materials such as pH buffers and lots of pre -made
standards are included in this requirement. Ref: Quality Assurance Policies for Field
Laboratories.
Comment: Implementation of the Traceability Log supplied by the auditor by email on 11/8/2012
will satisfy this requirement.
Temperature — Standard Methods, 2550 B - 2000
B. Finding: The laboratory is not meeting the required hold time.

Requirement: The hold time for Temperature is analyze (i.e. immediate). Ref: Code of
Federal Regulations, Title 40, Part 136; Federal Register Vol. 77, No. 97, May 18, 2012; Table
II.
Dissolved Oxygen — Hach 10360 (LDO)
Comment: The laboratory was citing Standard Methods, 4500 O G-2001 as the analysis method for
DO. However, the laboratory was performing an unapproved method (Hach 8166) for the analysis of
DO utilizing a Hach DR 850 colorimeter instead of a meter equipped with an electrode. North Carolina
Administrative Code, 15A NCAC 2H .0805 (g) (4) states: Each facility must have glassware,
chemicals, supplies, equipment and a source of distilled or deionized water that will meet the minimum
criteria of the approved methodologies. Notification of acceptable corrective action (i.e., confirmation
of the purchase and implementation of a new meter and LDO probe) was received by email on
10/29/12.
Comment: The laboratory's Attachment 1 has been updated with the approved Hach Method 10360
(effective 10/29/2012) to reflect the newly purchased/ implemented meter and LDO probe.
Total Residual Chlorine - Standard Methods, 4500-CI G - 2000
Comment: The laboratory is not verifying the instrument's factory set curve every 12 months. NC
WW/GW LC Approved Procedure for Field Analysis of Total Residual Chlorine states: Analyze a water
blank to zero the instrument and then analyze a series of five standards. The curve verification must
check 5 concentrations (not counting the blank) that bracket the range of the samples to be analyzed.
This type of curve verification must be performed at least every 12 months. The values obtained must
not vary by more than 10% of the known value for standard concentrations greater than or equal to 50
µg/L and must not vary by more than 25% of the known value for standard concentrations less than 50
µg/L. The overall correlation coefficient of the curve must be 2:0.995. Notification of acceptable
corrective action (i.e., confirmation that the factory set curve for TRC was verified for both meters
belonging to the laboratory) was received by email on 11/16/12. No further response is necessary for
this finding.
PAPER TRAIL INVESTIGATION:
The paper trail consisted of comparing field testing records and contract lab reports to Discharge
Monitoring Reports (DMRs) submitted to the North Carolina Division of Water Quality. Data were
reviewed for Chocowinity WTP (NPDES # NC0087491) and Richland Township WTP (NPDES #
NC0084808) for January, February and March 2012. No transcription errors were detected. The facility
appears to be doing a good job of accurately transcribing data.
V. CONCLUSIONS:
Correcting the above -cited findings and will help this lab to produce quality data and meet certification
requirements. Please respond to all findings.
