Describe the outfall point in detail using information from the Addendum
Outfall will be located on the applicants property where the pond meets applicants driveway. The effluent will
discharge next to the driveway utilizing rip rap for post ariation. Discharging directly to an unnamed tributary
feeding into the pond.

This is an existing permitted (WQ0002908) Non -Discharge permit that is loosing access to the adjacent property
where the existing sprayfield is located.
RO Recommendation: The RRO does not object to modifying the existing facility and the issuance of this
permit in accordance with the current policy of Single Family NPDES Discharge.
