Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to
the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between
North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007 (or as subsequently
amended).
This final permit includes no significant changes from the draft permit sent to you on
August 4, 2010.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings
(6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this
decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or Local

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES permits issued to this facility, whether for operation or discharge are hereby
revoked. As of this permit issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under
the permit conditions, requirements, terms, and provisions included herein.
UCS, Inc. is hereby authorized to:
1. Continue to operate an existing 0.3-acre waste stabilization lagoon followed by
chlorine disinfection. This wastewater treatment system is located at the UCS
Main Plant site (511 Hoffman Road, Lincolnton) in Gaston County.
2. Discharge from said treatment works at the location specified on the attached
map into the South Fork Catawba River, currently classified WS-IV waters in
the Catawba River Basin.

In the days following Mr. Bell's visit facility staff initiated a maintenance program to
remove excess vegetation. The program (which will be detailed in the forthcoming Storm water Pollution
Prevention Plan) requires continued (monthly) visual inspection and upkeep by facility staff. No
burrowing animals have been observed since the last one was eliminated during winter 2009.

In the days following Mr. Bell's visit the process of clearing thick, woody vegetation was
completed to the best of the machinery's capabilities without risking harm to it's operators. Contact with
a local landscape, berm maintenance expert has been established and ongoing conversations about
necessary actions that may be required will be detailed in a future company report. It is expected to be
recommended that UCS does not remove the observed tree to prevent future erosion and berm issues.

The forthcoming Storm Water Pollution Prevention plan will detail the "bucket -
stopwatch" measurement method for effluent flows. It will further detail the frequency and method of
recording those measurements as required by the permit.

5. Directions to Site: From Lincolnton travel south on Hwy 321. Take exit 20 and turn
right on Gastonia Highway (NC155) and travel approximately 0.2 mile to the intersection
with Hoffman Road (SR 1562). Travel on SR 1562 approximately 0.8 mile. UCS, Inc. is
located on the left (south) side of SR 1562.

PART IV - EVALUATION AND RECOMMENDATIONS
UCS, Inc., is applying for Permit renewal for the subject facility. It maybe necessary that
the lagoon be lined depending upon comments/recommendations from the Aquifer Protection
Section. No flow for this facility has been reported on the DMR since the year 2006. According
to the owner Jason Schwartz, this facility reduced tremendously the water use since 2006, the time
at which this facility was purchased.
Pending review and approval by the Western NPDES Program Unit, It is
recommended that the NPDES permit be renewed as requested.
