Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant
to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as
subsequently amended).
This final permit includes no changes from the draft permit sent to you on January 14, 2009.
Third Creek is listed as an impaired stream on North Carolina's 303(d) list. This means that the stream
does not meet all water quality standards. Although non -point sources are the main contributor to this
degradation, the Division will continue to monitor compliance at your facility. Should any additional
degradation of the stream be attributable to the discharge, the Division will take necessary steps,
including modification of the permit, to ensure protection of surface water quality.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty
(30) days following receipt of this letter. This request must be in the form of a written petition,
conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of
Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless
such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or
Local governmental permit that may be required.

On the basis of thorough staff review and applica-
tion of NC General Statute 143.215.1 and 15A NCAC
02H.0109 and other lawful standards and regula-
tions, the North Carolina Environmental Manage-
ment Commission proposes to Issue a National
Pollutant Discharge Elimination System (NPDES)

The NC Department of Transportation (P.O. Box

Rest Area in Iredell County. This permitted facility
discharges treated wastewater to the Camel
Branch in the Yadkin -Pee Dee River Basin. Cw-

David L Millsaps has applied for renewal of NPDES
permit NCO023191 for the Seven Cedars Mobile
Home Park W WTP in Iredell County. This Permit-
ted facility discharges.010 MOD of treated waste-
water to Third Creek in the Yadkin -Pee Dee River
Basin. Currently, total residual chlorine is water
quality limited. This Discharge may affect future
allocations in this portion of the Yadkin -Pee Dee
River Basin.
Hill Haven Residential Care. Inc (114 Hill Haven
Road. Statesville. NC 28677) has applied for re-
newal of NPDES permit NCO045012 for the Hill Ha-
ven Residential Care W WTP in Iredell County. This
permitted facility discharges treated wastewater
to an unnamed tributary to Third Creek in the
Yadkin -Pee Dee River Basin. Currently fecal
coliform, ammonia nitrogen and total residual
chlorine are water quality limited. This discharge
may affect future allocations in this portion of the
Yadkin -Pee Dee River Basin.
The Barium Springs Home for Children P.O. Box 1
Barium Springs. NC 28010] nos applied for renewal
of NPDES permit NC0045971 for ifs WWiP in
Iredell County. This permitted facility discharges
treated Wastewater into an Unnamed tributary, to
Duck Creek in the Yadkin -Pee Dee River Basin.
Currently BOD, ammonia nitrogen, dissolved oxy.
gen, feral coliform and total residual chlorine are
water quality limited. This discharge may affect
White allocations in this portion of the watershed
Mr. Homer Prevette has applied for renewal of per-
mit NCO077615 for the Homer's Truck Stop WWTP
in Iredell County. This permitted facilitydischarg-

The Permittee, Seven Cedars Mobile home Park is applying for renewal of its NPDES
permit to discharge treated domestic wastewater from the wastewater treatment system serving
the Mobile Home Park in Iredell County. The wastewater treatment plant appeared to be in good
operational condition during the time of the investigation.
Pending review and approval by the Western NPDES Program Unit, it is recommended
that the permit be renewed as requested.

It is a 0.010 MGD Waste Water Treatment Plant consisting of bar screen, aeration basin
with diffused air, clarifier with sludge return lines, V-notch Weir, gravel filtration tanks,
and aerated sludge holding tanks. The sludge tanks consist of four tanks: two 1000
gallon tanks and two 2000 gallon tanks. Bio Tech, Inc -Environmental Service (803) 796-
8925 removes our sludge from Plant.

Description of the sludge management plan for
Seven Cedars Mobile Home Park Waste Water Treatment Plant.
A settlebility test is conducted. When the settlebility reaches 45-50% the wasting sludge
process begins by decreasing the sludge return flow to the aeration basin. This allows the
sludge to build in the settling tank. After approximately 10 to 12 hours we close the
sludge return to the aeration basin and open the valve to the sludge holding tank. The
sludge is then pumped out of the settling tank and into the sludge holding tanks where
further settling occurs. The clear water which accumulates after further settling is then
pumped back into the aeration basin. When the sludge holding tanks are full Biotech Inc -
Environmental Service removes the sludge from the facility.
