Division of Water Resources' Central and Regional staff has reviewed the application package
received September 14, 2022. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on December
16, 2022.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through Q, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2).
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with an

1. Per [15A NCAC 02T .0604(c)], engineering design documents shall be signed, sealed, and dated
by a professional engineer. Engineer signatures and dates are present throughout the application
package; however, the seal is not showing on the application package. Below is an example of what
can be seen on each sealing location throughout the application package:

This is likely a matter of digitizing or resolution since the outline of the seal can be made out. Please
look at the original file and ensure that all seals are present. Then provide a new copy for review.

F. Soil Evaluation:
1. Per [15A NCAC 02T .0604(b)], a detailed soils evaluation shall be submitted. The figures
submitted are gray scale and appear grainy. This makes fine details difficult to see. Please resubmit
a higher resolution, color, copy of the soils report.
2. Additionally, a soil delineated map is required by the above rule. Please include one if not already
present.

G. Engineering Plans:
1. Per [ 15A NCAC 02T .0604(c)(1)], engineering plans for the entire system shall be provided. Within
the engineering plans, there appear to be multiple driplines missing as can be seen here:
I believe this is once again the result of digitizing and/or a glitch while transferring documents.
Please resubmit a higher resolution plan set.
2. Please note that the seals located on each plan sheet are missing, as is the case for the rest of the
permit application package.

I. Engineering Calculations:
1. Regional staff noted the following regarding the overall state of the field:
"The proposed disposal field has several large trees growing in it. Fill material should not be placed
within the drip line of these trees as it could result in tree death. An arborist can provide guidance on
the appropriate clearance to be left around the trees. This area should be removed from the available
disposal area calculation and accounted and compensated for in the loading rate calculations."
Please respond to the above concern and elaborate on whether the trees have been accounted for in
the design.
