ADDITIONAL REGIONAL STAFF REVIEW ITEMS See attached inspection report for additional comments and observations from the last site visit. Below are additional observations from reviewing the most recent aDDlication packaee: 1. Reported flows have decreased significantly over this calendar year. The apartment complex has successfully changed all water fixtures in the units in March 2022. 2. The freeboard for the 5 day upset tank was reported wrong, the actual freeboard capacity is 27.6ft. 3. The freeboard for the 14 day wet weather tank was reported wrong, the actual freeboard capacity is 28.5ft. 4. The pump and haul permit WQ0042770 has expired. 5. WQCSD0738 is the deemed collection system that serves this system. The deemed permit was issued because of SSO from one of the grinder pumps. 6. The MW parcels are incorrect. MW and MW3 are on 2900-22-8697. MW2 is on 2900-32-6210. 7. Attachment A requiring a site map. Will the site map on file work? One was not provided. 8. The flow reported in 06-16 IV.6 and 7 (pg 271 in LF) is not correct. The number of bedrooms used in the calculation is wrong, the permit description lists 894 bedrooms, 5 employees in the clubhouse, a pool, and a fitness center. Why did they use 859 bedrooms? 9. 06-16 IV.8 and 9 (pg 272) were not answered. 10. 06-16 V.1 (pg 273) does not include effluent values. Needed? 11. 06-16 V.1 La (pg 274) lists the volume of the EQ tank as 40,786 gallons. The cover letter/response to the add info lists the new volume as 49,274 gallons. What is the effective volume of the tank? 12. 06-16 V.1 Lc (pg 274) is incorrect. The permit description lists the UV unit as 130 gpm. 13. 06-16 VIIIA is still selected as no. They will not be producing effluent to reclaimed quality? 14. Several items on the drawings on pg 287-290 are incorrect. Key Note 2: Proposed 4" : 2" : 4" line from EQ tank to rotary screen? Will be replacing existing line? Key Note 7 and 8: The anoxic tank and bioreactor tank locations are switched. See attached drawing. There is a 4" round PVC drain. Key Note 21: Is this the supply line from the sump pump to the EQ tank? There are three pipes on the side of the EQ tank. Don Feller stated that the large pipe is influent, the medium pipe is from the 5 day upset, and the small pipe is an overflow to the sump tank. Is the small pipe actually the return from the sump tank? Key Note 35: There is no fence around the facility. Key Note 37: The generator is not where it is mapped. See attached Drawing. Key Note 41: Based on the drawings from the last application, this is a new proposed line. There is already a floor drain in the rotary brush screen room. When/why was this added? Are the plans on pg 403-406 old plans? 15. The O&M plan does not appear to be updated as you asked? 16. Is there going to be some way to activate the rotary brush screen when the 90% overflow is used? The wastewater levels in the post anoxic, bioreactors, and anoxic tanks control when the knife valve is opened and the rotary brush screen is activated. If wastewater is just entering the rotary brush screen without it actively moving it will likely clog the holes and push trash into the tanks. Is the assumption that if the overflow line is used that the knife valve and rotary brush screen are activated? Lon Snider and Jenny Graznak of the Winston Salem Regional Office were present. Eric Youngs, ORC, and Christy LAST NAME, Complex Manager, were present. A review of records, treatment units, storage units, and permitting changes was completed. Ms. Caudle requested certain records to be delivered electronically by January 3rd, 2023. To date these records have not been received. This review mostly reflected compliance with the subject permit. The following deficiencies were noted: Per Permit Condition V.1 "the Permittee shall provide adequate inspection and maintenance to ensure proper operation of the wastewater treatment and irrigation facilities." The following items were noted to need maintenance and repairs: A. The knife valve on the equalization tank was leaking. This was also noted in the previous inspection and in NOV-2020-PC-0330 dated July 28, 2020. Make the necessary repairs to stop the valve from leaking. B. Pump P-8001 B in the B treatment train was spraying wastewater when in use. This pump is used to mix wastewater between the pre -anoxic and bioreactor tanks. Mr. Youngs stated that the bearing in the drive shaft of the motor has failed and as a result wastewater is leaking around the drive shaft when in use. Mr. Youngs is only using that pump when on site. Make the necessary repairs the pump motor to prevent leaks and return to B train to normal operations. C. Mr. Youngs stated that the level transducer in the equalization tank is not operating correctly. Wastewater level in the tank is not being reported accurately to the control system. Mr. Youngs stated that a small amount of wastewater overflowed from the storage tank to the ground, but did not enter the storm drain. Per Permit Condition V.1 referenced above and Permit Condition 111.1 "... The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility." Make the necessary repairs to ensure proper operation of the treatment plant. D. The dissolved oxygen meter in the aeration/bioreactor in B train was not operating correctly at the time of inspection. Mr. Youngs stated it needs to be replaced. Replace the dissolved oxygen meter when possible. E. One of the UV disinfection units was not operational at the time of inspection. Mr. Youngs stated that there is an electrical problem in the circuit board and a new board is not available. Repair the current system or purchase an equivalent UV disinfection unit as both units must be operational. Per Permit Condition V.1 referenced above and Permit Condition 111.1 "the facilities shall be properly maintained and operated at all times." As a reminder, the unit as described in the permit description is a 130 gpm Severn Trent UV disinfection system. F. Mr. Youngs stated there is a leak somewhere on the forcemain to Zones 1-4. Zones 5 and 6 are still being irrigated. Mr. Youngs stated he was planning to investigate the forcemain when the students leave for winter break Repair the forcemain when possible. G. The embankment above the storage tanks has degraded more since the last inspection and the storm water diversion is not operating efficiently. There was evidence of soil eroding downslope and being deposited near the storage tanks. There was standing water next to the wet weather storage tank. Per Permit Condition V.1 referenced above and Permit Condition V.2 "the Permittee or their designee shall inspect the wastewater treatment and irrigation facilities to prevent malfunctions, facility deterioration and operator errors resulting in discharges..." Take measures to prevent further degradation of the embankment above the storage tanks and improve the stormwater diversions to prevent water from ponding around the storage tanks. The following item of concern was noted: • The 14-day wet weather storage tank had a few small leaks around the second band of metal plates from the bottom of the storage tank. This was also referenced in NOD-2021-PC-0506. Mr. Youngs stated that he is continuing to make repairs as needed. The nature of the tank and its location will cause leaks to be an ongoing problem. Continue to make repairs on the storage tank. • There have been weather related issues to removing screenings from the WWTP. Degradation of the access road during the summer caused the dumpster at the WWTP to overfill and be to heavy for the solid waste hauler to remove. The dumpster has yet to be emptied. Mr. Youngs is storing screenings in wheelbarrows in the rotary brush screen room for the time being. This may be an issue down the road as screenings were entering the floor drain to the sump tank. Other Observations: • Mr. Youngs is still managing the drip zones himself by making small repairs. Larger repairs are still contracted out. • A rain sensor has been installed. • The Spill Plan was posted in the treatment plant. • Anoxic Tank A has been temporarily patched. A larger drain valve was installed the cover the hole in the tank. There were no leaks visible around the valve. Mr. Youngs stated they are continuing to search for a permanent solution, but the location of the tank is making it difficult. • All meters and the irrigation system were calibrated on 12/2022 per the certification sticker on the flowmeters. • At the time of inspection, the high water alarm for the dose tank was activated and silenced. Mr. Youngs stated that the alarm will be activated as the plant is transferring effluent from the dose tank to the back wash tank, then to the wet weather storage tank. There is a three way valve that controls the direction of flow. To the dose tank, to back wash, or to wet weather storage. Mr. Youngs stated the high water alarm will call out a singular time when initially activated. • The mats on top of the storage tanks are still in place. There was no wind at the time of inspection allowing for stable freeboard readings. Mr. Youngs stated that the mats will be removed when a safe option is available. • There is a floor drain in the rotary brush screen room that is piped to the sump tank. • The water supply wells for the apartment complex are housed in the two buildings on the WWTP access road. • A resident in the complex is assisting Mr. Youngs in the operation of the treatment plant. The resident is measuring pH and TRC and making visual observations at the WWTP. • The sump tank motor was replaced recently. Ms. Caudle recommended that the sump tank also be pumped when the lift stations are pumped as solids were visible in the tank. The high water alarm was operational. • Effluent has been analyzed for NO2+NO3 for several month. NODs have been sent separately as part of monthly report reviews. Mr. Youngs stated that NO3 is now separated and analyzed individually per Attachment A. • NO2+NO3 and NH3 have been above the ground water limits in the effluent for several months since the membrane bioreactors were replaced. Past inspectors stated that this will happen when the filters are new and NO3 will decrease in effluent as the membrane bioreactors are seasoned. • The two large lift stations and 23 small grinder pump stations were not inspected. • The bioreactors have aeration. • DO: 0.82 ppm pH 6.41 • Sludge is removed about twice a year. • Both membrane bioreactors have been replaced. • Freeboard in 5-day upset: 22ft. Freeboard in 14-day: 15ft. • When weather allows, the irrigation system is run on a timer. When the dose tank is emptied the system will pull effluent from the wet weather storage tank. The only float in the system is the high water float that transfers water to the wet weather tank. • Sludge wasting starts at a certain time each day. Some sludge is wasted during each flush of the MBR. Sludge is wasted until the Desired Sludge Amount is reached. This allows for only a small percentage of the total biomass to be wasted to leave the system with each flush.
