Lnviromnental Services, Inc. is snbmitting five (5) copies of the enclosed PCN ou behalf of NCDOT for
proposed improvements to NC 53 in Ovslow Cowity. The proposed widening and intersectiov
improvements will improve traffic safety. The proposed improvements have been minimized as much as
practicable, but will result in unavoidable impacts to waters of die United States, including 131 feet of
permanent im�acts to jurisdictional streams and 0.14 acre of impacts to jurisdictional wetlands.
Compensatory mitigation is provided for impacts lo jurisdictional weUands and streams. USACE has
reviewed the site and are in agreement with the jurisdictional wetland and stream deliueation.

Proposed roadside ditches were designed to meet grass swale critena (maximum 3:1 side slopes). Grass swale data at the permit sites have been
provided where applicable.
The typical section from -L-70+50 to 104+50 is proposed to include a 2'-6" wrb and gutter in order to avoid impacts to a recently constructed 12" water
line, avoid R/W impacts, and avoid impacts to identifed National Register Historic PropeAies. There is no opportunity to provide any type of treatment
for stortnwater draining to the UT to Blue Creek at approximately 87+50. Blue Creek is not listed as an impaired water on the 303(d) list.

The proposed project will make improvements to a 1.8 mile section of NC 53 and the NC53/SR 1109
(Holly Shelter Road) intersection. The project will include the following:
Widening of NC 53 from 2 lanes to 3 lanes between a point 0.2 mile west of the NC53/SR 1214
(Foy Lockamy Road) intersection to the NC53/SRi116 (Onslow Pines Road) intersection;
Road realignment and widening at the NC 53/SR 1109 (Holly Shelter Road) intersection;
The existing 1@24" RCP (L=46') on a UT to Southwest Creek (Site 1 on attached drawings) is to
be replaced with a 1@66" x 51" CAPA (L=80');
The existing 1@4' x 4' RCBC (L=46') on a UT to Blue Creek (Site 4 on attached drawings) is to be
replaced with a 1@6' x 6' RCBC (L=68');
The project work will consist of grading, widening, drainage improvements, pavement widening,
roadway construction, pavement markings, and pavement markers. This work will be conducted using

surface water exists and that it is subject to the mitigation rules may request a detennination by the Director. A
request for a determination by the Director shall be referred to the Director in writing c/o Amy Chapman, NCDWR

Individuals that dispute a determination byNCDWR or Delegated Local Authority that "exempts" a surface water
from the mitigation rules may ask for an adjudicatory hearing. You may obtain the petition form from the office of
Administrative hearings. You must file the pe[ition with the office of Administrative Hearings within sixty (60)
days of receipt of this notice a�id the date the affected party (including downstream and adjacent landokmers) is

projects are located on the east end of the current widening project. The SHPO issued "No CommenY'
letters for both projects.

A map review and site f le search was conducted at the Offi[e of State Archaeology (OSA) on Friday, July 20, 2012.
No previously recorded archaeological sites are situated within the APE, but one prehistoric resource (310N277) is
located adjacent along the northern side of NC53 near/beyond [he western project limits. Recorded by an amateur
status individual in February of 1979, the si[e form maintained at the Office of State Archaeology (OSA) contained
absolutely no information regarding [he sires environmental or altural contexts. The resource remains
unevaluated in terms of National Register of Historic Places (NRHP) eligibility. All indications place this dubious
resource on an elevated, well-drained upland land sur/ace several hundred feet west of lhe intersection
improvement locale and ou[side any potential impact activi[y.
An examination of NRHP, State Study Listed (SL), Locally Designated (LD), Determined Eligible (DE), and Surveyed
Site (SS) properties utilizing resources available on the NCSHPO website determined that none of the above
designated properties with potential archaeological subsurface deposits were located in the defined APE or dose
nearby.
Furthermore, topographic maps, his[orical maps, USDA soil wrvey maps (Ba6), and archaeological/historical
reference materials were inspec[ed by the cuitural resource specialist to evaluate both environmental and cultural
factors that may have led to prehistoric/historic occupation of the APE. Aerial photographs and the Google Street
View map application were also used to lend a first-hand, on-ground perspective of the land surfaces within and
surrounding the APE to assess the level of impacts and disturbances that may have resWted in the degrada[ion or
destruction of formerry unknown archaeological deposits wi[hin the project cons[ruction footprint.

According to engineering design plans subml[ted by the division environmental officer, the proposed construction
work is to occur within lhe bounds of the existing and highly dislurbed righ[-of-way corridors of the Iwo subject
roads. The projectjurisdictional fea[ures map illustrates that the APE ground surfaces between NC53 and SRI109,
as well as on the northern side of NC53 near the westem project Iimits, are characterized by wetlands. Aerial
photographs and the Google Street Viev✓ map appllcation further demonstrate the disturbed and modified
character of the area. Because previously recorded site 310N277, according to OSA maps and recently analyzed

Briefdescription of revrew activities, results ofreview, and co»clusions: HPOWeb reviewed on 27 July
2012 and yielded no NR, SL, LD, DE, or SS properties in the Area of Potential Effects (APE). Onslow
County current GIS mapping, aerial photography, and tax information indicated a predominantly wooded
APE with several unexceptional mid-twentieth-century resources (viewed 27 July 2012). Three nearby SS
noted in the review request (Eule Gurganus House (ON 170), Morton-Dixon House (ON 395), and
Thomas Jefferson Morton House (ON 391)) stand outside the APE, as do two bridges: Number 18 on NC
53 and Number 71 on SR 1109 (Holly Shelter Road), Google Maps "Street View" confirmed the absence
of critical historic structures/landscapes in APE (viewed 27 ]uly 2012).
No architectural survey is required for the project as currently defined.
