We issue this permit pursuant to the requirements of North Carolina General Statute 143- 215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007, or as subsequently amended. Changes to Your Previous Permit. Based on your renewal application received April 28, 2012, we have revised your permit with the understanding that you have made no significant alterations to your wastewater treatment facilities since last permitted (2006). For renewal therefore, we have only updated your facility description and map, and added parameter codes to the effluent monitoring pages for your convenience [see Supplement to Cover Sheet and permit, Section A. (1.)]. Change from the Permit Draft. In the permit draft the Division inadvertently excluded instream monitoring (U & D) for the parameter Dissolved Oxygen [00300]. Please continue instream monitoring for Dissolved Oxygen. We have corrected this oversight in this permit final, consistent with your previous permits. The Division regrets any confusion this may have caused your organization. If any parts, measurement frequencies, or sampling requirements contained in this permit are unacceptable, you have the right to an adjudicatory hearing, upon written request submitted within thirty (30) days after receiving this letter. Your request must take the form of a written petition conforming to Chapter 150B of the North Carolina General Statutes, and you must file it with the office of Administrative Hearings, 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such a demand is made, this permit shall be final and binding. This permit is not transferable except after notifying the Division of Water Quality. The Division may require permit modification, or revocation and re -issuance.
