Dear Mr. Johnson:
Division of Water Resources' Central and Regional staff has reviewed the application package
received on August 29, 2022. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on November
28, 2022.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through 0, or failure to provide the additional information on or before the above -requested
date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2).
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed, and dated (where needed), with an

C. Application (RWS-R 02-21):
1. Section III — Flow Information:
a. There was no reclaimed water produced during the previous calendar year (2021) per the
NDMR forms submitted. The listed average flow of 300,000 GPD appears to be
incorrect and the current hydraulic capacity for the facility should be 0% based on this
information. Please confirm this.
b. Please clarify the wastewater composition percentages as they do not add up to 100%
and there has been an added industrial percentage.
2. Section IX — Utilization Fields:
a. The previous permit lists one utilization field in Attachment B, but there is nothing listed
in this section. Please confirm that the irrigation area has not changed from the previous
permit, which was listed as 87.0 acres.
D. Attachment A — Site Map:
1. A review of our records shows that a major modification was completed for the facility on
December 8, 2008; therefore, Attachment A is required. Pursuant to 15A NCAC 02T .0105(d),
please submit a site map with updated aerial imagery pursuant to the requirements in 15A NCAC
02U .0201(d).
E. Attachment B — Signature Authority Delegation:

provide that as well.
