reason:
B1. This permittee does not match the entity that is shown on the Gaston County SE&SC. The SE&SC says Tod Farlow is
the responsible party, while the NCG01 says the responsible party is Helmsman Homes, LLC.
If Todd Farlow is the correct responsible party/permittee, his name needs to be listed as responsible entity/permittee on
the NCG01's Tab B1 and Tab E as well as the NOI certification form, uploaded to Tab D9.
If Helmsman Homes, LLC is the correct responsible party/permittee, the FRO form will need to be modified to show
Helmsman Homes, LLC as the responsible party on Part B1. That FRO form will then need to be sent to Gaston County,
so they can update the responsible party shown on their SE&SC form.
Please submit a new NOI. To auto -populate the new NOI, select'Yes' for the first question & enter NOI number 114889.
