Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached final NPDES discharge permit. This permit is issued pursuant
to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as
subsequently amended).
Please take note that all facilities required to have nutrient removal for phosphorous must be classified
one grade higher. [See 15A NCAC 08G .0302(e), effective December 1, 2006;
http://h2o.enr.state.nc.us/tacu/wwrules.html]
"Any system receiving a classification of Grade II Biological Water Pollution Control System
that is required to achieve nutrient reduction is assigned the classification of Grade III
Biological Water Pollution Control System." (Effective December 1, 2006)
Subsequently, sampling requirements must change from weekly, to 3/week per 15A NCAC 2B .0508.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings
(6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this permit
shall be final and binding.
Please take notice that this permit is not transferable. This permit does not affect the legal requirements
to obtain other permits which may be required by the Division of Water Quality or permits required by the
Division of Land Resources, Coastal Area Management Act, or any other Federal or Local governmental
permits which may be required.

In compliance with the provisions of North Carolina General Statute 143-215.1, other lawful standards
and regulations promulgated and adopted by the North Carolina Environmental Management

Onslow County
to receiving waters designated as Northeast Creek in the White Oak River Basin in accordance with
effluent limitations, monitoring requirements, and other conditions set forth in Parts I, II, III and IV
hereof.
This permit shall become effective December 1, 2007.
This permit and authorization to discharge shall expire at midnight on June 30, 2012.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked, and as of this issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under
the permit conditions, requirements, terms, and provisions included herein.
Carolina Water Service, Inc. of NC is hereby authorized to:
1. Continue to operate an existing 0.125 MGD wastewater treatment system with the following

I am writing to respond to your letter dated August 22, 2007, objecting to the change from a Class
II facility to a Class III facility. Unfortunately, we are unable to consider your request not to change the
monitoring frequencies. All facilities required to have nutrient removal for phosphorous must be

PART V - EVALUATION AND RECOMMENDATIONS
Note: This facility is required to meet a 2mg/1 phosphorus limit on a weekly composite sample. This
is achieved by the chemical addition and simultaneous precipitation of the phosphorus and activated
sludge in the clarifier unit. The phosphorus is then removed from the system with the waste
activated sludge. This phosphorus removal process does meet the intent of recent rule changes to
increase the classification of wastewater systems that achieve nutrient reduction one level. It should
be noted that the addition of this chemical is a fairly straightforward procedure and does not
significantly increase the operational complexity of this facility.
The Wilmington Regional Office recommends reissuance of Permit No. NC0032239 in accordance
with the White Oak River Basin Plan permitting strategy and the permitting guidance attached
provided no significant adverse public comment is received.
