Dear Mr. Brooks:
A letter of request for Authorization to Construct was received October 3,
1988, by the Division and final plans and specifications for the subject project
have been reviewed and found to be satisfactory. Authorization is hereby
granted for the construction of dual 1500 gallon septic tanks with baffles, a
tablet feed type chlorinator and all associated valves, pipes and appurtenances
to become a part of West Lincoln High School wastewater treatment facilities
located in Lincoln County.
This Authorization to Construct is issued in accordance with Part III,
paragraph B of NPDES Permit No. NC0041246 issued January 12, 1984, and shall
be subject to revocation unless the wastewater treatment facilities are
constructed in accordance with the conditions and limitations specified in
Permit No. NC0041246.
The Permittee must employ a certified wastewater operator in accordance
with Part III paragraph C of the referenced permit.
The sludge generated from these treatment facilities must be disposed of in
accordance with General Statute 143-215.1 and in a manner approvable by the
North Carolina Division of Environmental Management.
The Mooresville Regional Office, phone no. 704/633-1699 shall be notified
at least twenty-four (24) hours in advance of operation of the installed
system so that an in -place inspection can be made. Such notification to the
regional supervisor shall be made during the normal office hours from 8:00 a.m.
until 5:00 p.m. on Monday through Friday, excluding State Holidays.

In the event the facilities fail to perform satisfactorily in meeting its
NPDES permit effluent limits, the Permittee shall take such immediate corrective
action as may be required by this Division, including the construction of
additional wastewater treatment and disposal facilities.
The septic tank shall be checked regularly and pumped out as required. The
tablet chlorinator shall be checked every week until the actual usage rate is
established and thence on a monthly basis. Tablets shall be added so as to
provide proper chlorination.
