Dear Ms. Rose:
The final plans and specifications for the subject project have been reviewed
and found to be satisfactory. Authorization is hereby granted for the construction
of modifications to the existing 2,500 GPD septic tank -sand filter wastewater treat-
ment facility to consist of relocating the existing post chlorination facilities to
-the effluent discharge line of the. existing surface Band filters. to serve Haywood.
Moncure Community Healthh Center. -
.' This is a Clans I Wastewater Treatment Facility and the person in responsible
charge must hold a valid Grade I Certificate.
'This Authorization to Construct shall be subject to revocation unless the waste-
water treatment facilities are constructed in accordance with the conditions and
limitations specified in Permit No. NC0030384.
One (1) set of approved plans and specification is being forwarded to you.
