North Carolina General Statutes and the Coastal Stormwater rules require that this property be covered under a
stormwater permit. Failure to maintain a permit subjects the owner to assessment of civil Penalties. More

Dear Permittees:
On November 14, 2014, the Wilmington Regional Office of the Division of Energy, Mineral and Land Resources (DEMLR)
inspected the two subject projects in New Hanover County to determine the status of compliance with their respective
State Stormwater Permits in preparation for a modification to SW8 050612 and a transfer in permit ownership. Please find
copies of the completed forms entitled "Compliance Inspection Report" attached to this letter for each permit, which
summarizes the findings of the recent inspection.

1. The project is permitted for a maximum of 32,025sf of impervious area. I he August iu, 2uuo permit allocatec
17,237sf of impervious area. I understand that the current proposal is to add 2,356sf of impervious area bringing.
the total to 20,735sf which is within the allowable amount. If the approved plans show all of the standard,
required items (such as a wetlands note, a note that roof drains will be directed to the system, etc) I do not
anticipate that there will be any significant problems permitting the modification.
2. The following documents are needed for the permit transfer to the new owner.
A. Name/Ownership Change Form: Please submit the Name/Ownership Change form signed by both the
�uQ outgoing/current permittee and the incoming/proposed permittee. I have mailed this letter to both
parties, and you may each submit separate signed forms to the address at the bottom of this letter if that
its easier for you.
b. .$ignature Authority Documentation: 134 Newton, LLC is a new LLC and the Articles of Incorporation on
b the Secretary of State's website do not list the names of the managers. Please provide documentation
that Mr. Anthony is a manager of 134 Newton, LLC.
✓c. Deed: Please submit a copy of the deed showing that 134 Newton, LLC is the property owner.
