No fee is required for this type of authorization
Also, some information that is needed for Buffer Authorizations that is sometimes missed
• Send three copies of the application to the address shown at the bottom of the application
• ✓The owner needs to sign the application where indicated because the Buffer Authorization will
go to the owner If the agent /consultant of the owner wants to sign the application, the
agent /consultant must fill out an agent authorization form (example attached)
• he stream needs to be shown on the plan AJ'

Xhow the two zones of the riparian buffer on the plan as measured from the top of the
streambank Zone 1 is measured 30 feet from the top of the stream bank, and Zone 2 is
measured from 30 feet to 50 feet These riparian buffers parallel the stream pattern f
