Durham County tax records list you as the owner of the subject property. We are contacting you to
determine the status of a wastewater disposal system at the site. That system was originally covered under a
state NPDES discharge permit, one of over 430 such systems in Durham County.
An audit of expired / unresolved files noted that coverage for your system expired in July 2007. The
Division needs information from you to determine if coverage under NCG550000 [the General Permit for
domestic wastewater] is still necessary.
➢ If your property still has a wastewater system like the ones described in the enclosed Technical
Bulletin, you must renew the subject CoC. Complete the enclosed form and submit it to me at the
address on the form.
➢ If you are not sure what type of system your property has, contact Mack Wiggins in the NC DENR
Raleigh Regional Office at 919 791-4200. He [or other staff members] can help you determine if
you should renew your CoC.
➢ If you know that your property no longer discharges wastewater, contact me at the address or
phone number listed below to request rescission of the CoC.
I have enclosed the pages from our files that describe the system. The last Certificate of Coverage was
issued to Michael Sevier in July 2002. If you have any questions, please contact me at the telephone number

We continue to receive the annual permit Invoice for the permit !ICG550020 at
653 Donlee Dr. Durham, NC 27712.
That property was sold In 2005. We have previously notified you -and forwarded the
Permit Invoices to the new owners at the Donlee address.
Attached please find a copy of Settlement Statement with monetary amounts covered
for privacy reasons. I've also attached a business card with the name of our realtors for
the sale.
Also, please be advised, that at the time of sale- a Spanish interpreter was utilized by
the buyers, Leonclo and Edelmlra Valladares.
