Dear Mr. & Mrs. Holder:
In accordance with your application for discharge, the Division is forwarding herewith the subject
Certificate of Coverage to discharge under the subject state-NPDES general permit.. Authorization is hereby
granted for the construction and operation of a 480 gallons per day wastewater treatment system consisting of a
septic tank with a minimum capacity of 1000 gallons, adjustable cap distribution box, 318 square foot (6'X53')
Advan Tex fixed -film media pod, recirculation tank, and UV disinfection unit to discharge treated wastewater into
unnamed tributary to Aho Branch classified WS IV waters in the Watauga River Basin. All elbow piping must be
of the long sweeping type. All cleanouts are to be housed in meter boxes below the surface. This system must be
at least 10 feet from the dwelling and property lines and at least 100 feet from water supply wells on and off the
site. The system must also be constructed and located above a 100-year flood.
If any parts, measurement frequencies or sampling requirements contained in this general permit are
unacceptable to you, you have the right to submit an individual permit application and letter requesting coverage
under an individual permit. Unless such demand is made, this decision shall be final and binding.
The Authorization to Construct is issued in accordance with Part III, Paragraph 2 of NPDES Permit No.
NCG550000, and shall be subject to revocation unless the Wastewater treatment facilities are constructed in
accordance with the conditions and limitations specified in Permit No. NCG550000.
In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions,
the Permittee shall take immediate corrective action, including those as may be required by the Division, such as
the construction of additional or replacement wastewater treatment or disposal facilities.
The Winston-Salem Regional Office, telephone number (336) 771-5000, shall be notified at least forty-
eight (48) hours in advance of operation of the installed facilities so that an in -place inspection can be made.
Such notification to the regional supervisor shall be made during the normal office hours from 8:00 a.m. until
5:00 p.m. on Monday through Friday, excluding State Holidays.

Upon completion of construction and prior to operation of this permitted facility, a certification must be
received certifying that the permitted facility has been installed in accordance with the NPDES Permit, the
Certificate of Coverage, this Authorization to Construct and the approved plans and specifications. Mail the
Certification to the NPDES Unit, 1617 Mail Service Center, Raleigh, NC 27699-1617. A copy of the approved
plans and specifications shall be maintained on file by the Permittee for the life of the facility.
Failure to abide by the requirements contained in this Authorization to Construct may subject the
Permittee to an enforcement action by the Division of Water Quality in accordance with North Carolina General
Statute 143-215.6A to 143-215.6C.
This CoC is not transferable except after notice to the Division. The Division may require modification
or revocation and reissuance of the CoC. Part II, E.4. addresses the requirements to be followed in case of change
of ownership or control of this discharge.
The issuance of this permit does not preclude the Permittee from complying with any and all statutes,
rules, regulations, or ordinances which may be required by the Division of Water Quality or permits required by
the Division of Land Resources, the Coastal Area Management Act or any Federal or Local other governmental
permit that may be required.

4. Other Special Items: This is a single family NPDES General Permit application. It is a
site that currently has a failing subsurface system permitted through the county and is in
need of an alternative solution. Due to lot size, spray or drip systems are not an option.
A site visit was performed and it appears that Mr. Holder's's system is not functioning as
it should and that sewage is seeping up onto his lawn.

Enclosed please find information supporting an application for an AdvanTex discharge system for the
residence of John and Margaret Holder located in Watauga County, North Carolina. The following
information is included with this letter:
- Notice of Intent (NCG550000). See Attachment A
- $50 check payable to North Carolina DENR.
- Letter from Watauga County indicating no options for surface disposal are allowed.
See attachment B.
- Letter requesting coverage under the General Permit. See Attachment C.
- Letter requesting Authorization to Construct. See Attachment D.
- Soils Evaluation report by Dr. Jeff Vaughan. See Attachment E.
- Receipt showing septic tank has been pumped within 12 months. See Attachment F.
- Plans and Specifications for the proposed AdvanTex system. See Attachment G.
- Pictures of the site that show the drainage/discharge area. See end of this letter.
- Certification Letter from AdvanTex Distributor (AQWQ) regarding performance of system. See
Attachment H.
As part of the Notice of Intent, the feasibility of alternatives to discharging must be addressed. The
following alternatives are to be evaluated:
- Connection to a Regional or Municipal Sewer Collection System
- Letter from the local or county health department describing the suitability or non -suitability of
the site for all types of wastewater ground adsorption and innovative non -discharge
systems. Document the repair potential of the failed system.
- Investigate Land Application such as spray irrigation or drip irrigation.
Connection to a Regional or Municipal Sewer Collection System
Connection to a sewer collection system is not feasible for this residence. The closest sewer collection
system is — 4 miles from this residence. Based on a cost of $125/ft this would equate to an approximate
cost of $2,640,000 to connect this residence to a regional sewer system.
Letter from the local or county health department describing the suitability or non -suitability of
the site for all types of wastewater ground adsorption and innovative non -discharge systems.
Document the repair potential of the failed system.
A letter from the Watauga County Health Department is attached with this letter. The letter from Watauga
County states there is not enough usable space to repair the failing system. As seen in attachment B, the
letter suggests the best option for Mr. Holder is to apply for a NPDES discharge permit.
Investigate Land Application such as spray irrigation or drip irrigation.
Per the soils evaluation by Dr. Jeff Vaughan, the site is lacking three major criteria that will not
accommodate a subsurface drip or spray wastewater system;
-adequate space of provisionally suitable soil
-appropriate topography
-Aho Branch would prevent the use of a drip and spray wastewater system
7Q10 Flow Estimate
The United States Geological Survey (USGS) was contacted to determine the 7Q10 flow estimate for the
proposed wastewater discharge point. The proposed system will discharge into Aho Branch. Based on
information from the USGS the 7Q10 flow estimate is 0.08 cfs.
Picture of the Site
A picture of Aho Branch ( at the proposed discharge point) can be seen in Figure 1. Aho Branch will
receive the treated discharge from the AdvanTex system.
Site Setbacks
Due to the lot size, lot configuration, existing house location and presence of Aho Branch, the required
setbacks can not be met. The required setbacks are shown on the site drawing. There is no area
available to meet the setbacks and site the system. Therefore, we are requesting a waiver of the
applicable setbacks. The location of the treatment components will meet the 50' setback from Aho
Branch.

to be constructed.

application, record, report, plan or other document filed or required to be maintained under Article
21 or regulations of the Environmental Management Commission implementing that Article, or
who falsifies, tampers with or knowingly renders inaccurate any recording or monitoring device or
method required to be operated or maintained under Article 21 or regulations of the Environmental
Management Commission implementing that Article, shall be guilty of a misdemeanor punishable
by a fine not to exceed $25,000, or by imprisonment not to exceed six months, or by both. (18

Dear Mr. and Mrs. Holder:
This letter is in reference to the failing septic tank system located on your property at 108
Meadowbrook Lane. Effluent has been surfacing over the nitrification field for several
months. There is nowhere on your property to effect a repair of the system. You have
been unable to find an area off -site for a ground absorption sewage system. I believe your
best option is to apply for a NPDES discharge permit for a wastewater treatment system.
A discharge permit was issued in the past under similar circumstances for another lot in
this subdivision. The appropriate state agency (NC Division of Water Quality), not the
local health department, gust approve any plans for the treatment system. If you have
any further questions regarding this matter, please call me at 828-264-4995.

SUBJECT: Soils Evaluation for 108 Meadowbrook Lane, Blowing Rock NC
A preliminary soils evaluation was conducted for an approximately 1 acre piece of property
located at 108 Meadowbrook Lane in Blowing Rock, NC. An in -field investigation was
conducted on the property on November 1, 2007. A home is already located on the site and the
current septic system is malfunctioning. The property is unsuitable for a surface or subsurface
septic system due to an unsuitable landscape position, too little available space, and close
proximity to a surface water. The site is a likely candidate for a surface water discharge system.
