I have attached my inspection report regarding this visit. Mr. Joe Valdez from Guilford County DHHS accompanied me
to the property. Mr. Briones did not speak English very well and Mr. Valdez helped me interpret. I relayed that if he did
request the permit be rescinded he would have to reapply for a new permit, or he would have to request that DHHS
re-evaluate the site for subsurface treatment.
The system was designed for forested spray irrigation and the entire property has been cleared. A new system may have
to be designed anyways for the parcel as it is now.
I have also attached my correspondence with Ms. Briones.

Inspection Summary:
On November 3, 2022, Division of Water Resources staff Caitlin Caudle completed an announced compliance inspection.
Joe Valdez of Guilford County Department of Health and Human Services accompanied Ms. Caudle to the site. Mr. Ismael
Hernandez was present during the entire inspection.
There is an existing septic tank on the property. The rest of the designed system has not been installed. There is no one
residing in the single -wide currently on the property and there is an existing well. Mr. Hernandez is using the property to
keep his animals for the time being.
A septic tank was installed previously, but was capped and disconnected from the trailer with the previous permit renewal.
Ms. Caudle sent information to Mr. Hernandez sister about rescinding the current permit as he does not desire to keep the
permit.
The existing design recommended that the trees/vegetation on the property be maintained for optimal waste integration. The
property has since been cleared with no trees present. If a system was to be installed under the current permit, it would
likely have to be redesigned.

Hi Annel,
As Joe Valdez and I discussed with Ismael today, he can request to rescind the permit from my office.
An email must be sent our central office to Nathaniel Thornburg([e-mail]) and state that
Ismael would like the permit to be rescinded and that the system has not been built. Include the permit number:
W Q0014446.
Just a note, if a regular septic system (subsurface) cannot be installed on the land a surface system will have to be
installed. Which is what the current permit is for. If a surface system is needed in the future, a new system will have to
be designed and new permit issued. That typically involves hiring and engineer and soil scientist. But if Ismael does not
plan on building a house on the property a permit is not needed.
