Dear Mr. Triplette:
Final plans and specifications for the subject project have been reviewed and
found to be satisfactory. Authorization is hereby granted for the construction of
a 0.026 MGD wastewater treatment facility to consist of :influent bar screen and flow
distribution box, dual 0.013 MG package extended aeration plants, tertiary treatment
facility with two (2) rapid sand filters, post chlorination facilities, flow measur-
ing device, and 1970 lineal feet of 8-inch gravity sewer to serve the Western North
Carolina Conference of the Pentecostal Holiness Church.
This is a Class II Wastewater Treatment Plant and the operator in responsible
charge must hold a valid Grade II Certificate.
This Authorization to Construct shall be subject to revocation unless the waste-
water treatment facilities are constructed in accordance with the conditions and
limitations specified in Permit No. NC0046809.
Also, enclosed is a copy of WPC Form #50 "Cost of Wastewater Treatment Works."
This form is to be completed and returned to this office within thirty (30) days
after the project is completed.
One (1) set of approved plans and specifications is being forwarded to you.
