This email is to notify you that your request to rescind COC No. NCC191763 for the project Brockton Drive Lake and
Dam Project - Phase 2 was not approved for the following reason(s):
Richard Kelly is the legally responsible individual. His name goes on the legally responsible person line of the NOT
Certification Form. Gilles Bellot can digitally sign the eNOT, but Gilles must wet sign the NOT Cert Form, and print his
name and title on the bottom line of the form. This also allows one to also digitally sign the eNOT. Questions, contact
