Phase II Addition to Hawthorne Wastewater
Treatment Plant

Dear Mr. Roberts:
A letter of request for Authorization to Construct was received February 14,
1985, by the Division and final plans and specifications for the subject project
have been reviewed and found to be satisfactory. Authorization is hereby granted
for the construction of a 0.10 MGD wastewater treatment facility consisting of
the following additions to the existing 0.05 MGD plant: a 50,000 gallon capacity
aerated tank for flow equalization and emergency wastewater storage; a splitter
box; a second 0.05 MGD extended aeration plant with a 50,000 gallon capacity
aeration tank and dual 250 CFM blowers, a 7,500 gallon capacity sludge holding
tank, 8,333 gallon capacity clarifier, a 1,100 gallon capacity chlorine contact
tank with tablet type chlorinator; a recording effluent flow meter and
totalizer; equipment for chemical removal of phosphorous by the "Odophos"
process including two (2) 3000 gallon concrete tanks lined with polyethylene
for chemical storage and dual chemical feed pumps toserve the entire plant;
and all other necessary piping, valves, fittings, and appurtenances to serve
Hawthorne Subdivision.
This Authorization to Construct is issued in accordance with Part III
paragraph C of NPDES Permit No. NC0049662 issued June 20, 1984, and shall be
subject to revocation unless the wastewater treatment facilities are constructed
in accordance with the conditions and limitations specified in Permit No. NC0049662.
The Permittee must employ a certified wastewater operator in accordance
with Part III paragraph D of the referenced permit.

The sludge generated from these treatment facilities must be disposed of in
accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina
Division of Environmental Management. Details describing the method of disposal
shall be submitted to the Division at least thirty (30) days prior to the initial
disposal of sludge.
The Raleigh Regional Office, telephone number 919/733-2314 shall be
notified at least twenty-four (24) hours in advance of start up of the installed
facilities so that an in -place inspection can be made of said system prior
to. operation. Such notification to the Regional Supervisor shall be made
during the normal office hours 8:00 a.m. until 5:00 p.m. on Monday through
Friday, excluding State Holidays.
In event the facilities fail to perform satisfactorily in meeting its
NPDES permit effluent limits, the Permittee shall take such immediate cor-
rective action as may be required by this Division, including the construction
of additional wastewater treatment and disposal facilities.
