OBTAIN GRADING/EROSION CONTROL PLAN APPROVAL FROM NORTH CAROLINA DEMLR AND OBTAIN AN APPROVED NC GENERAL PERMIT NCG01 FROM NORTH CAROLINA DEMLR. 2. NOTIFY NC DEMLR 48 HOURS PRIOR TO ANY LAND DISTURBING ACTIVITY. 3. A PRE -CONSTRUCTION CONFERENCE MUST BE HELD WITH THE NC DEMLR / MILLS RIVER AT LEAST 48 HOURS PRIOR TO BEGINNING ANY LAND DISTURBING ACTIVITIES. THE OWNER, DESIGN ENGINEER AND CONTRACTOR MUST BE PRESENT AND HAVE OBTAINED THE STORM WATER PERMIT, STAMPED APPROVED PLANS AND APPROVAL LETTER ALL APPLICABLE JURISDICTIONS BEFORE CALLING TO SCHEDULE THIS MEETING. 4. CALL FOR UTILITY LOCATES (PRIVATE AND PUBLIC). 5. THE PRE -CONSTRUCTION MEETING SHALL TAKE PLACE ONSITE. INSTALL RAIN GAUGE AND JOB BOX. A COPY OF THE APPROVED PERMITS AND ONSITE SWPPP SHALL REMAIN ACCESSIBLE (ONSITE) DURING CONSTRUCTION. CITY OF HIGH POINT ENGINEERING INSPECTOR AND/OR A DEMLR REPRESENTATIVE SHALL BE PRESENT AT THE PRE -CONSTRUCTION MEETING. 6. FOLLOWING THE PRE -CONSTRUCTION MEETING, THE CONTRACTOR SHALL DETERMINE AND MARK THE LIMITS OF DISTURBANCE AND PROVIDE TREE PROTECTION WHERE NECESSARY. CONTACT THE ZONING ADMINISTRATOR TO REVIEW TREE PROTECTION (IF APPLICABLE). 7. INSTALL RAIN GAUGE AND JOB BOX. ALL PERMIT APPROVALS, DRAWINGS AND WEEKLY EROSION CONTROL INSPECTIONS SHALL BE KEPT ONSITE AND READILY AVAILABLE FOR REVIEW BY ENGINEER OR COUNTY/STATE INSPECTORS. 8. THE CONTRACTOR IS RESPONSIBLE FOR LOCATING ALL EXISTING UTILITIES PRIOR TO COMMENCING WITH DEMOLITION AND/OR CONSTRUCTION ACTIVITIES. THE CONTRACTOR SHALL ENSURE PROTECTION OF THE PUBLIC AT ALL TIMES THROUGHOUT CONSTRUCTION ACTIVITIES THROUGH THE USE OF TEMPORARY CHAIN LINK FENCE, DRIVEWAY BARRIERS, AND SILT FENCING TO INDICATE THE LIMITS OF THE CONSTRUCTION AREA WHILE MAINTAINING ACCESS TO THE ACTIVE FACILITY. THE OWNER MAY HAVE SPECIAL REQUIREMENTS TO ACCOMMODATE ACCESS TO SPECIFIC LOCATIONS OF THE FACILITY AND THE CONTRACTOR SHALL OBSERVE THOSE REQUIREMENTS ACCORDINGLY. 9. INSTALL PERIMETER CONTROL BMPS INCLUDING SILT FENCE, TEMPORARY CONSTRUCTION ENTRANCE/EXIT, AND INLET PROTECTION AT EXISTING INLETS (IF APPLICABLE) PRIOR TO ANY LAND DISTURBING ACTIVITIES AND DEMOLITION. THE PERIMETER CONTROLS SHALL BE MAINTAINED AND ADJUSTED THROUGHOUT CONSTRUCTION AND REPLACED AS NECESSARY. THE CONTRACTOR IS SOLELY RESPONSIBLE FOR THE PREVENTION OF SEDIMENT TRANSPORT FROM THE DISTURBED AREA(S) DURING CONSTRUCTION ACTIVITIES, AND SHALL IMPLEMENT ADDITIONAL BMPS THROUGHOUT CONSTRUCTION AS NECESSARY IN ORDER TO PREVENT OFF -SITE IMPACTS. 10. REMOVE TREES AND PERFORM DEMOLITION REQUIRED TO CONSTRUCT STORM WATER MANAGEMENT POND AND OUTLET PIPES. 11. INSTALL STORM WATER MANAGEMENT POND, OUTLET STRUCTURE, SKIMMER, BAFFLES AND FILTER BERM PER PLANS. 12. PROVIDE MATTING ON INTERIOR AND EXTERIOR POND SLOPES PER DETAIL. 13. INSTALL INLET PROTECTION AS INDICATED ON THE PLANS. 14. PROVIDE TEMPORARY DIVERSION DITCHES/BERMS TO ROUTE RUNOFF INTO STORM WATER MANAGEMENT POND. PROVIDE STONE CHECK DAMS AS NECESSARY TO MINIMIZE EROSION WITHIN DRAINAGE SWALES. PROVIDE CONTINUOUS MAINTENANCE AS REQUIRED. 15. ONCE THE DEMOLITION AND INSTALLATION OF BMPS FOR PHASE 1 IS COMPLETED, THE CONTRACTOR SHALL NOTIFY THE ENGINEER AND NC DEMLR TO HAVE AN ONSITE MEETING TO DETERMINE THAT PHASE 1 SEDIMENT AND EROSION CONTROL DEVICES HAVE BEEN INSTALLED ACCORDING TO THE APPROVED PLANS. CONTRACTOR SHALL RECEIVE APPROVAL FROM ENGINEER BEFORE BEGINNING PHASE 2 SEDIMENT AND EROSION CONTROL AND ASSOCIATED GRADING.
