1. Attach a vicinity map (i.e. street map) clearly showing the location of the property with
respect to local landmarks such as towns, rivers, and roads.
2. Provide a detailed site plan showing property boundaries and proposed locations of
vegetation clearing, structures (buildings, retaining walls, docks, impervious surfaces,

etc.), rip rap, excavation or dredging below Full Pond/ Normal Water Level elevations,,
and construction access corridors. You may use the diagram provided at the end of this
application form.
3. Attach a photograph of the shoreline/ buffer proposed to be stabilized. (Include a scale
of some sort- a yardstick, shovel handle, etc.)

Directions to site - please include road names and numbers, landmarks, etc.: From
Highway 150 in Mooresville, go west past Marshall.Steam Station and through the Terrell
(small town),stoplight. Continue west on 150, then turn right,on Little Mountain'Road.
Follow Little Mountain Road 0.75 miles, then turn right on'Landmark Drive. The property is
on the right as you come out of the curve (see diagram)

(residential, undeveloped, etc.): residential (vacation home, circa 1965
6. Property size (acres): 0.48 acres

8. Describe proposed work (include discussion as to how hardening of shoreline has been
avoided, or'why it is necessary): replace,existing concrete block retaining wall (which is
damaged and falling over) with a new rip -rap retaining wall. Shoreline will' be sloped to
accommodate the rip -rap wall.
