On September 13, 2022, the Mooresville Regional Office (MRO) performed an inspection of the
Remediation System at the Former Community Cash and Carry facility located on Hwy 801 in
Mt. Ulla, to determine compliance with remediation activities as per permit #W10300466.
Enclosed is a copy of the inspection report.

Inspection Summary:
On September 13, 2022, MRO staff performed a Compliance Evaluation Inspection for the Former Cash and Carry GW
remediation system under permit # WI0300466.
During the site visit, O&M records were reviewed from 1/2022 through 8/2022. The effluent flow meter was changed on
4/25/2022. The Air Stripper was cleaned on 7/22/2022.
During the site visit, MRO staff requested any waste manifests for drummed waste generated by the system be sent to the
MRO via email. MRO staff also requested that the GW analytical data for September 2022 GW or an equivalent report
including GW analytical data be sent to MRO via email.
A review of the GW analytical data was presented during the site visit. The lab results for 2022 and the historic GW
analytical data since the system returned to service indicate the reduction of lead on well MW-12 and MW-8. It was
reported that RW-1 was pumping fine. However, the pump rate for RW-4 was at a pumping rate that appears to be too high
for the well to recover adequately. It may be less stressful for the treatment system to back down on the pump rate of the
RW-4 well to prevent strain on the pump and allow the GW in the well to achieve better recovery.
The system does not currently utilize telemetry notifications. If a system failure occurs, it would not be found until the next
monthly ORC visit.

RW-4 well has been reported to be set at a pumping rate too high for the well to allow for adequated GW
recovery.
Comment: It may be beneficial to reduce the pump rate for well RW-4 to put less stress on the well motor
and allow for GW recovery to occur.
