A review of available records and treatment facilities was completed. Mark Hunt and Brian King of Whites Trucking were present during the entire inspection. This review reflected compliance with the subject permit. The following items of concern were noted: • To date, a permit renewal application has not been submitted per Permit Condition 1.1. The current permit expired on March 31, 2022. An electronic copy of the renewal application was provided on August 29, 2022. Once completed, the application can be submitted here: https://edocs.deq.nc.gov/Forms/NonDischarge-Branch-Submittal-Form-Ver2. • Per Permit Condition IV.4 a maintenance log shall be maintained at this facility. Visual observations of the treatment facility and any preventative maintenance (inspections, cleaning, changing equipment) shall be recorded. An inspection log was not maintained for the system, but it was in good condition and well maintained. Some maintenance information was available in the form of residuals removal invoices and emails regarding replacement of the carbon filter. Moving forward, please maintain a singular inspection log describing any observations or repairs.
