Description of projects area and activity: This permit authorizes placement of fill and in -stream structures for stream
stabilization/restoration activities. Impacts authorizes total 226 linear feet of an unnamed tributary to Smith Mill
Creek.

Your work is authorized by the above referenced permit provided it is accomplished in strict accordance with the
attached conditions, above noted special conditions, and your submitted application and attached information dated
July 27, 2022. Any violation of the attached conditions or deviation from your submitted plans may subject the
permittee to a stop work order, a restoration order, a Class I administrative penalty, and/or appropriate legal action.
This verification will remain valid until the expiration date identified below unless the nationwide/regional authorization is
modified, suspended or revoked. If, prior to the expiration date identified below, the nationwide/regional permit authorization
is reissued and/or modified, this verification will remain valid until the expiration date identified below, provided it complies
with all requirements of the modified nationwide/regional permit. If the nationwide/regional permit authorization expires or is
suspended, revoked, or is modified, such that the activity would no longer comply with the terms and conditions of the
nationwide/regional permit, activities which have commenced (i.e., are under construction) or are under contract to commence
in reliance upon the nationwide/regional permit, will remain authorized provided the activity is completed within twelve months
of the date of the nationwide/regional permit's expiration, modification or revocation, unless discretionary authority has been
exercised on a case -by -case basis to modify, suspend or revoke the authorization.
Activities subject to Section 404 (as indicated above) may also require an individual Section 401 Water Quality Certification.

GENERAL NOTES:

3. EROSION AND SEDIMENT CONTROL PRACTICES MUST BE INSTALLED, INSPECTED AND MAINTAINED IN ACCORDANCE WITH FEDERAL, STATE AND LOCAL REGULATIONS.
4. ALL EQUIPMENT MUST BE CLEANED REGULARLY AND MAINTAINED TO BE FREE OF FUEL, OIL AND LUBRICANT LEAKS.
5. FUELING SHALL BE PERFORMED AT LEAST 100 FEET FROM FLOWING WATER.
6. THE CONTRACTOR SHALL PROTECT SITE FEATURES BEYOND THE GRADING LIMITS. CONTRACTOR SHALL REPAIR ANY DAMAGE AND RETURN WORK AREAS TO PRE -PROJECT CONDITIONS.
7. EQUIPMENT SHALL OPERATE FROM THE TOP OF BANK AS MUCH AS PRACTICAL AND SHALL TAKE PRECAUTIONS TO LIMIT DOWNSTREAM SEDIMENT MIGRATION.
8. BASE FLOW SHALL DIVERTED AROUND WORK AREAS USING BARRIERS WHEN EQUIPMENT MUST WORK IN THE CHANNEL. WORK SHALL BE SCHEDULED DURING PERIODS OF LOW FLOW.
9. WORK SHALL BE STAGED SUCH THAT NO MORE STREAM BANK IS DISTURBED THAN CAN BE STABILIZED WITH SEED, MULCH AND MATTING AT THE END OF EACH WORKDAY.
10. DISTURBED UPLAND AREAS MUST BE STABILIZED WITH SEED AND MULCH WITHIN 7 DAYS OF REACHING FINAL GRADES.
11. SILT FENCE SHALL BE INSTALLED ON THE STREAM SIDE OF ANY STOCKPILES.
12. PLANT MATERIALS SHALL BE INSTALLED DURING THE DORMANT SEASON, GENERALLY DECEMBER THROUGH MARCH.
CONSTRUCTION STORMWATER INSPECTIONS:
1. THE CONTRACTOR IS RESPONSIBLE FOR KEEPING DAILY RECORDS OF RAINFALL EVENTS IN AN ON -SITE PERMIT BOX.
2. EROSION AND SEDIMENT CONTROL PRACTICES MUST BE INSPECTED AT LEAST ONCE PER 7 CALENDAR DAYS AND WITHIN 24 HOURS OF ALL RAINFALL EVENTS GREATER
THAN 0.5 INCH PER 24 HOUR PERIOD.
3. NECESSARY MAINTENANCE OR REPAIRS MUST BE COMPLETED IMMEDIATELY UPON IDENTIFICATION OF THE PROBLEM.
