Dear Mr. Lambert:
In accordance with your permit major modification request received June 10, 2022, and subsequent
additional information received September 13, 2022, we are forwarding herewith a modified Attachment B
certified September 14, 2022, to Davie County for the subject residuals management program.
Modifications to the subject Attachment are as follows:
Addition of Fields NC-DV-008-10 and -11 in Davie County, for a total of 29.01 acres.
The removal of Field NC-DV-008-08 in Davie County, for a total of 17.09 acres.
The modified Attachment B shall be effective from the date of certification until September 30,
2028 (i.e., expiration date of the current permit), shall replace Attachment B of Permit No. WQ0010583
issued June 17, 2022, and shall be subject to the conditions and limitations as specified therein, as well as
those specified in the above-referenced permit. Please replace Attachment B of Permit No. WQ0010583
that was issued June 17, 2022 with the modified Attachment B. Please note that the Permittee shall
submit a renewal application no later than April 3, 2028.
Please note that only the maps for new land application sites are attached to this letter. It is the
complete permit is created by inserting these documents into the
previously issued permit.
If any parts, requirements, or limitations contained in this certification are unacceptable, the
Permittee has the right to request an adjudicatory hearing upon written request within 30 days following
receipt of this permit. This request shall be in the form of a written petition, conforming to Chapter 150B
of the North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail
Service Center, Raleigh, NC 27699-6714. Otherwise, this certification shall be final and binding.
