The Division of Water Resources (Division) has performed a preliminary review of a 401 Water Quality
Certification application submitted for the above referenced project. The Division has determined that your
application is incomplete and cannot be processed. The application is on -hold until all of the following
information is received:
The Plan sheet enclosed with the application is not of sufficient scale to be usable.
Provide a plan sheet, preferably with a scale of 1 inch = 50 feet, clearly depicting the following:
• Project boundary
• Building, parking area, entrance and road way footprints (i.e., all impervious surfaces)
• Proposed culvert location
• All existing streams and wetlands
Include a north arrow and scale
2. A cross section of the culvert is required to confirm the installation of the culvert will meet
Condition 13 of Water Quality Certification 3890 — Placement of Culverts... In Waters
3. Describe the installation techniques for the culvert to confirm the installation of the culvert will
meet Condition 6 of Water Quality Certification 3890 — Work in the Dry
4. Describe how stimi water runoff will be transported to confirm stormwater management will
conform with Attachment A requirements of Water Quality Certification 3890
5. Provide culvert pipe type (e.g., CMP, plastic corrugated, etc.)

If all of the requested information is not received in writing by October 29, 2014, the Division will be unable to
approve the application and it will be returned. The return of this project will necessitate reapplication to the
Division for approval, including a complete application package and the appropriate fee.
Please respond in writing within 30 calendar days of receipt of this letter by sending three copies of all of the
above requested information to the 401 & Buffer Permitting Unit:
