Dear Mr. Atkins:
Division of Water Resources staff Caitlin Caudle conducted a review of the 2021 Residuals
Annual Report. This review reflected non-compliance with the subject permit. The following
violation was noted:
1. Per Permit Condition IV.8 "...an annual report shall be submitted on or before
March 1st." An annual report was submitted to the Winston Salem Regional Office on
June 9, 2022, by Michael Frazier after Ms. Caudle contacted him. This is a
violation of the subject permit. Moving forward please be sure that all Annual
Reports are submitted by March 1st
A compliance inspection will be scheduled within the next 12 months. Please refer to the
enclosed inspection report for additional comments and observations. If you have any

Division of Water Resources staff Caitlin Caudle completed a review of the 2021 Residuals Annual Report. This review

No application activities were reported in the 2021 calendar year. All residuals were hauled to the Mount Airy WWTP.
