4. Project Description and History
4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:
The proposed project is located in the Town of Eden, in Rockingham County. Mebane Bridge Road crosses over the Dan River. Land use in the vicinity of the project is wooded along the
river, with commercial and some residential in the area. On the north side of the river is a water treatment facility.

4h. Explain the purpose of the proposed project: *
The primary purpose of the project is to remove a structurally obsolete bridge. Bridge No. 780233 was built in 1924 and has been closed to traffic for approximately 20 years. The bridge is
a historical structure but poses safety issues due to spalling concrete falling into the river, as well as trespassing occurring on the closed bridge. A sewer line currently attached to the
bridge will also be relocated as part of this project in advance of the bridge removal since the sewer line anchors are failing and sewage leakage into the river is a concern if the line is not
moved.
The purpose of this application is specifically to conduct a subsurface investigation (geotechnical boring) for the removal of Mebane Bridge and the sewer line relocation.
41. Describe the overall project in detail, including indirect impacts and the type of equipment to be used: *
The proposed boring in the Dan River will be performed utilizing a floating aluminum barge platform with an ATV drill rig mounted to the deck. The barge is self-propelled utilizing outboard
marine motors. The barge will be launched from the existing boat ramp located approximately 0.5 mile upstream from the existing bridge. The proposed boring location is approximately
100 feet downstream of the existing bridge and there is expected to be 6-10 feet of river depth at this location. The barge will be secured at the boring location with aluminum spuds
temporarily set into the bottom of the river at the four corners of the barge drilling platform. Outboard motors will not run while the barge is secured. The boring work will take approximately
1-2 days to complete, depending on encountered soil and rock conditions.
The barge will be equipped with a spill kit and other safety equipment to manage any fluid leaks from the drilling operation, should they occur. The boring will be drilled using rotary wash
and rock coring methods. Temporary steel casing will be advanced into the subsurface to provide borehole stability and to provide a conduit for performing the rotary wash drilling, SPT
sampling, and rock coring. At the completion of the boring any drilling spoils will be returned to the subsurface via the temporary steel casing, and the tooling and spuds will be removed
and the barge will be moved back to the boat launch area for recovery.

1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project:
Drilling equipment will not drive through any jurisdictional stream and the barge will be launched from an existing boat ramp. Drilling equipment will not
drive through any jurisdictional wetlands. The boring work will only take approximately 1-2 days to complete, depending on encountered soil and rock
conditions. The bore hole will only be approximately four inches in diameter, resulting in a small impact footprint. By securing the barge with spuds rather
than other larger anchoring devices, the amount of sedimentation and stream bed impact from securing the barge will be minimized. Outboard motors
will not be in use once the barge is secured, which will prevent accidental fish strikes by the blades when the boring work is being conducted.
1 b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
A temporary steel casing will be advanced into the subsurface to provide borehole stability and to provide a conduit for performing the rotary wash
drilling, standard penetration test (SPT) sampling, and rock coring. This will prevent any sediment from escaping the bore hole into the river and will
separate the boring equipment from any accidental contact with fish. Although not quantified, the casing will also provide some level of sound
attenuation during drilling. At the completion of the boring, any drilling spoils will be returned to the subsurface via the temporary steel casing (preventing
excess sedimentation into the river), the tooling and spuds will be removed, and the barge will be moved back to the boat launch area for recovery. The
barge will be equipped with a spill kit and other safety equipment to manage any fluid leaks from the drilling operation, should they occur.

IPAC, NCNHP, field surveys. Bald eagle foraging habitat present, but no individuals or nests identified within the PSA or within 660 feet of the PSA;
survey completed January 28, 2022. Roanoke logperch, Atlantic pigtoe, James spinymussel surveys completed June 21, 2022. One Roanoke logperch
identified, no mussels found. After consultation with USFWS, it was determined that the small scope of the boring work would result in a Biological
Conclusion of MANLAA for all species. However, for the bridge removal/sewer line replacement, formal Section 7 consultation is anticipated, with a
MANLAA for mussels, but a MALAA for logperch for the future work outside of the borings. Please see attached USFWS concurrence for the boring
work and Freshwater Fish and Mussel Survey Report.

The bridge itself was determined eligible for the National Register. An Effects Meeting is required and was scheduled for August 25, 2022. Please see
the attached Effects Required form.
