However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on September 28, 2022. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through F, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2). Please reference the subject application number when providing the requested information. Residuals Land Application Program (FORM: RLAP 06-16) Application: 1. No comments. B. Land Application Site Certification (FORM: LASC 06-16) Form: 1. An updated site map was provided to remove areas from field NC-DV-008-11 due to unsuitable slopes, which reduced the applicable acres for this field from 7.21 acres to 6.03. Please provide an updated LASC that confirms this change. 2. In accordance with 15A NCAC 02T .1104(c)(1)(B), setback maps shall be provided to include a delineation of the compliance and review boundaries. Per the June 9, 2011 Map Guidance for Residuals Land Application Fields (Non -Dedicated) memo, this can be achieved through a footnote indicating the delineation of the compliance and review boundaries instead of the line drawn on the map. Please provide a delineation of the compliance and review boundaries on updated maps. The soil profile provided for field NC-DV-008-12 lists the seasonal high water table (SHWT) at 15". This field is also in the 100-yr flood elevation, meaning this field shall be incorporated or injected within a 24-hour period following land application to be compliant with 02T .1109(a)(1)(E). However, in accordance with 02T .1109(a)(1)(I), the minimum vertical separation of the SHWT and the depth of residuals application is 12". In this case, the depth of residuals would be the depth of incorporation. In accordance with 02T .1102(21), incorporation shall mean the mixing of residuals with topsoil to a depth of 4". To maintain a minimum separation of 12" to the depth of incorporation, the minimum depth to the SHWT shall be 16" if incorporation is required. Based on the provided soil profile, the boring location would not be suitable for land application. Please provide an updated delineation of Field 12 to include only areas that meet this 16" depth to water table that has been certified by a Licensed Soil Scientist to maintain compliance with 02T . 1 109(a)(1)(E). This new land application area shall be shown on an updated site map, and the acreage change shall be shown on an updated LASC. C. Landowner Agreement (FORM: LOA 06-16) Form: 7. No comments. D. County Board Notification (FORM: CBN 06-16): In accordance with the table in Item B of the Instructions for form LASC 06-16, all new fields shall include a CBN for each county in which proposed land application sites are located. This includes mailing the notification letter, source facility summary, land application site summary, and site location maps to the appropriate County Board by certified mail with a return receipt requested (in accordance with Item C of the Instructions for form CBN 06-16), and a proof of submission (in accordance with Item D). Please submit a completed and executed CBN 06-16 form for all counties with new proposed land application fields with a proof of submission to the relevant County Board(s).
