The application for SW8100315 has finally made its way to me I am also cc'ing Mr Baysden in this email Reviewing this
application, I lust need a couple things from you to issue this permit I know we are waiting on the engineer for the other
submittal, and when SW8031120 is submitted, these are going to be apart of that application as well
The application lists Mr Everett as chief but the North Carolina SOS registration does not show any listing officers
Please provide official documentation that lists Mr Everett as chief of Pender EMS and Fire, Inc Also, the previous
issued permit was made out to Pender Volunteer EMS and Fire, Inc and NC Secretary of State has documented that
official name change from Fender Volunteer EMS and Fire, Inc to Ponder EMS and Fire, Inc I have attached a permit
update form that needs to be completed and notarized for this documentation in our file records
Once I have this information, I can process this renewal Please let me know if you have any additional questions or

today This is for Pender EMS and Fire, Inc 805 Ridgewood Avenue, Burgaw NC 28425.
The other property located at 14388 NC HWY 210, Rocky Point, NC 28457 will be delayed awaiting on the new
survey from the engineer He said he was unable to get to it prior to the new year However, I have a call into
his office to check when they will get to it

Effective August 1, 2013 the State Stormwater program has been transferred from the Division of Water Quality
(DWQ) to the Division of Energy, Mineral and Land Resources (DEMLR). All previous references to DWQ will
remain in older stormwater permits issued prior to August 1, 2013 until they are modified, transferred, or
renewed. Please note that this letter references DEMLR as the Division responsible for issuance of the permit.
State Stormwater Management Permit #SW8 031120 and 4SW8 100315 have expired or will expire on the dates
listed below. This is a reminder that Permit renewal applications are due 180 days prior to their expiration per
I SA NCAC 2H.1045(3). North Carolina General Statutes and the Coastal Stormwater rules require that this
property be covered under a stormwater permit. Failure to maintain a permit subjects the owner to assessment of
civil penalties.
