Attachment A Certification
Wilkes County
In accordance with your permit major modification request received May 13, 2022, and subsequent
additional information received August 24, 2022, we are forwarding herewith a modified Attachment A
certified August 26, 2022, to U.S. Biosolids, Inc. for the subject residuals management program.
Modifications to the subject Attachment are as follows: Addition of the Dallas WTP (Permit No.
NC0082694/Water System No. NCO136065 — T01) as an approved residuals source, with a maximum
annual dry tonnage of 290 dry tons per year (DT/yr).
The modified Attachment A shall be effective from the date of certification until February 28, 2027
(i.e., expiration date of the current permit), shall replace Attachment A of Permit No. WQ0038071 issued
August 5, 2020, and shall be subject to the conditions and limitations as specified therein, as well as those
specified in the above -referenced permit. Please replace Attachment A of Permit No. WQ0038071 that
was issued August 5, 2020 with the modified Attachment A. Please note that the Permittee shall submit
a renewal application no later than September 1, 2026.
If any parts, requirements, or limitations contained in this certification are unacceptable, the
Permittee has the right to request an adjudicatory hearing upon written request within 30 days following
receipt of this permit. This request shall be in the form of a written petition, conforming to Chapter 150B
of the North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail
Service Center, Raleigh, NC 27699-6714. Otherwise, this certification shall be final and binding.
